Legendary Fisherman: Part 1 - S2-E19
Other mistake: During the duel with Marik's hunters, in the shot where Kaiba says "I hope you realise this is just the beginning of what my deck has in store for you", the spell card Pot of Greed can be seen in the monster zone on Kaiba's duel disk. First, Kaiba already activated this card so it should be in his graveyard. Second, since this card is a spell card, it should be in his spell and trap card zone and not his monster zone. (00:04:00)
Legendary Fisherman: Part 1 - S2-E19
Other mistake: When Kaiba has summoned Obelisk the Tormentor, a spell card can be seen on the short hunter's duel disk, but it is placed in the monster zone of the duel disk and not in the spell and trap card zone. (00:05:51)
Legendary Fisherman: Part 1 - S2-E19
Other mistake: When Mako has Flying Fish attack Joey directly, the trap card Torrential Tribute, which Mako had activated earlier, can be seen on his duel disk instead of the monster card Flying Fish. (00:16:35)

Legendary Fisherman: Part 1 - S2-E19
Other mistake: When Joey summons Alligator Sword, the bottom half of its face is coloured green. The card illustration for the Alligator Sword monster shows the bottom half of its face to be coloured yellow, while the top half is green. (00:17:50)
Legendary Fisherman: Part 1 - S2-E19
Other mistake: When Mako has his Amphibian Beast attack Joey's Alligator Sword, Joey activates Skull Dice, dividing Amphibian Beast attack points by four (from 2600 to 650). The attack then results in this monster being destroyed, because it was weaker than Alligator Sword, and Mako loses 700 life points because of it. However, Alligator Sword had 1500 attack points, meaning Mako should have lost 850 life points, due to there being an 850 point difference between the two monsters, and not 700 life points. (00:19:10)
Other mistake: When Umbra says "as I activate a magic card called Masked Doll," a spell/magic card can be seen in the monster card zone on his duel disk and not in the spell/trap card slots where it should be. Also, the card appears to be Mask of Brutality, which was activated by Lumis and not him. (00:16:32)
Other mistake: When Yugi suspects that the card Lumis placed face down is a trap card, the spell card Mask of the Accursed that Lumis just activated can be seen in the monster card zone on Yugi's duel disk. This card should not be in the monster card zone as it is a spell card and should be in the spell/trap card slots. (00:03:03)
Other mistake: When Yugi draws the Beast of Gilfer card, there is a shot focusing on Yugi, which then pulls out to reveal Kaiba looking at him. In the shot, the Magnet Warrior card can be seen on Yugi's duel disk in attack mode, even though it should be in defence mode. This monster remains in attack mode for the rest of the duel. (00:03:35)
Other mistake: On Lumis' turn, after Kaiba attacks with Blue-Eyes White Dragon, he places one monster face down in defence mode and sets two cards. When he does this, he is shown placing the monster card on his duel disk, but his hand never moves afterwards. The other two cards are set without him moving his hand. (00:05:25)
Let the Finals Begin! - S2-E32
Other mistake: When Yugi, Joey and Mai show the security guards their Locator Cards, they are all shown to have seven cards in their hand, even though they are supposed to have only six. (00:04:02)

Rage of the Egyptian Gods - S2-E36
Other mistake: When Yugi is looking at his cards, the monster card Cyber Jar can be seen towards the top right-hand side of the shot. Cyber Jar is a spell card in this shot and not a monster card. (00:07:38)
Awakening of Evil: Part 2 - S2-E38
Other mistake: When Joey sacrifices Battle Warrior to summon The Legendary Fisherman, the Battle Warrior monster card cannot be seen on his duel disk. This occurs right before The Legendary Fisherman is summoned, even though he was never shown removing the Battle Warrior card from his duel disk. (00:14:22)
Awakening of Evil: Part 2 - S2-E38
Other mistake: When Joey summons Insect Queen to the field, Jinzo can be seen in defence mode to the right of shot, but the monster card cannot be seen beneath it. Whenever monsters are in defence mode, the monster card is always shown beneath it, just like Insect Queen and the Legendary Fisherman at that same moment, but with Jinzo, no card can be seen. (00:17:30)
Awakening of Evil: Part 3 - S2-E39
Other mistake: At the start of the episode, the attack and defence points of each monster on the field are displayed. Insect Queen is shown to have 2400 attack and defence points when shown on the display. Insect Queen has 2200 attack points and 2600 defence points. (00:02:13)

Awakening of Evil: Part 3 - S2-E39
Other mistake: When Joey convinces Trinity not to leave the duelling field, there is a shot of Trinity saying, "You've got it, big brother," while Joey does a fist bump. In the shot, Jinzo can be seen on his duel disk, even though it was destroyed earlier in the episode. (00:06:45)
A Duel with Destiny: Part 1 - S2-E44
Other mistake: Kaiba activates the spell card Shrink, which halves his monster Vorse Raider's attack points. When the attack point display appears, it begins counting down from 1950 (even though Vorse Raider has 1900), and the attack points lower to 950 (which is 1000 points). Technically, the attack points were right to go down to 950 because Vorse Raider does have 1900 attack points, but the display was wrong to show 1950 attack points. (00:13:10)
