Continuity mistake: When Peter enters the cockpit and accidentally kills the pilots the cockpit looks small enough so that it could only fit Peter and the pilots in their seats. However, when he turns around to leave the cockpit looks big enough to fit an entire group of people.
Continuity mistake: When Karen Black is first shown, her blue uniform is sleeveless, showing her white shirtsleeves. After a cut to the "Dawson's Creek" kids, Peter says "It was a movie in the seventies," and Black's outfit has sleeves. Later, when she is helping people off the plane, her uniform once again is sleeveless.
Continuity mistake: When Peter is on the phone talking to Death the color of the napkin on the table changes from red to a brownish color.

Continuity mistake: Death has the TV remote asking how old the TV is. As Lois talks, he puts it down, then it suddenly disappears altogether.
Continuity mistake: As Quagmire and Cleveland are talking to Peter, the hat Cleveland is wearing disappears and reappears between cuts.
Answer: Lacey Chabert was the actress who originally voiced Meg in early episodes. She left of her own accord due to being in school and other acting work; Seth MacFarlane has stated there was no tension with her leaving, but it makes for a handy joke for the show.
