Continuity mistake: While Bart is sleeping, there is only one upright shoe on his nightstand. However, just as Homer wakes him up, a second shoe suddenly appears lying on the same nightstand next to the first one. (00:00:33)
Continuity mistake: When Homer wears the "Ralph 08" shirt, the first time the text appears with an apostrophe ("RALPH '08") and in a smaller size; later, in another shot, the apostrophe disappears and the text is shown in a much larger size. This difference is also seen on Bart's shirt. (00:16:21)
Treehouse of Horror XVIII - S19-E5
Continuity mistake: When Marge shoots Chief Wiggum, the arrow goes through his tie. When they roll on top of him, the arrow isn't through the tie anymore.
Smoke on the Daughter - S19-E15
Revealing mistake: When Lisa turns on the light while dressed in her Angelica Button costume, her mouth disappears for a single frame.
Character mistake: Before their first performance, the director of "La Boheme" wishes Homer good luck. To anyone in professional theatre, doing so is the cardinal sin of theatrical superstition and is never done; the appropriate phrase is "break a leg."
Continuity mistake: In the surgeon's office there are three certificates on the wall with ribbons on them. In the first shot there are two red ribbons and a blue one but in the shot where Homer stands up to read the bill all the ribbons are red.
I Don't Wanna Know Why the Caged Bird Sings - S19-E4
Factual error: During Dwight's attempted bank robbery, he gets hit with a couple of dye packs, then appears a few shots later totally clean. The dye packs banks use are notoriously difficult to wash off once they're triggered; even if Dwight did manage to clean himself up, there would still be remnants of dye on him.
Character mistake: When Mr. Burns and Smithers are walking through the morgue, the sign on the wall is incorrectly spelled "Hospitial Morgue."
Love, Springfieldian Style - S19-E12
Continuity mistake: In the third story of the episode, Bart and Nelson throw Martin into the Jukebox despite it never being opened. When he is trapped inside the box the glass is firmly in place as well.
