Wrecked - S6-E10

Continuity mistake: The episode begins the morning after Buffy and Spike first have sex. That night, Willow and Amy go out again to find Rack's place, and this cuts directly to Willow waking up the next morning. But later in the episode, Spike makes repeated references to what happened between him and Buffy "last night", when in fact it was two nights ago.

Once More, With Feeling - S6-E7

Revealing mistake: During the song-and-dance number "They Got The Mustard Out", if you look closely at the dancers, the man in green to camera right of the second row is horribly out of step with the music, and with the rest of the group. The spell makes everyone dance far better than they naturally would (Tara loses the clumsiness she often displays, for example), so it is a mistake to have someone under the spell dancing so badly.

Hush - S4-E10

Continuity mistake: The first night that the Gentlemen float through the college, they are on the 2nd floor one minute (all the room numbers start with 2), then they turn a corner and are suddenly outside room 118, a floor below.

Hush - S4-E10

Revealing mistake: In the fight scene at the clock tower, the first time Riley is knocked to the ground he kicks one of the strait-jacket demons in the face - his foot misses, but the demon still reacts as if hit.

Ghost Ship (2002)

Factual error: A ship as large as the Antonio Graza going down that fast would drag down everything around it too, yet Epps (within about 50 metres of the boat as it sinks) doesn't even have to struggle to escape its pull. In reality, she would have drowned. (01:20:25)

Innocence (2) - S2-E14

Continuity mistake: When Buffy is told that Angel has become Angelus, she mentions "I should have known. When I saw him at the house he was different", but her earlier conversation with him took place not at the house but at his apartment. This isn't just a strange turn of phrase - the scene was originally shot at the Summers house, but was changed to Angel's apartment for more emotional resonance.

Earshot - S3-E18

Continuity mistake: When Buffy is trying to read Angel's mind, his chain goes from being inside to outside his vest between shots.

Charmed (1998)

Power Outage - S3-E7

Continuity mistake: During the training drill, Piper fumbles for a knife on a table in front of her. In the shot of the table, she loses hold of the knife as she pulls her hand up, but when we cut to a long shot, it is back in her hand.

Charmed (1998)

Power Outage - S3-E7

Revealing mistake: When Astral Prue kicks Leo/Balthazar in the face during the training drill near the start of the episode, her foot misses his face yet he still reacts as if hit.

Charmed (1998)

Ex Libris - S2-E19

Continuity mistake: After the ghost chases Gibbs out of his pawn shop, and the police arrest him, his shirt goes from being on fully to half-off between shots (particularly noticeable when Tyra's father tells him to burn in hell).

Charmed (1998)

Ex Libris - S2-E19

Continuity mistake: When Phoebe is talking to the ghost in the library, the librarian goes from writing to glaring at her and back again between various shots.

Passion - S2-E17

Deliberate mistake: After Jenny pushes the janitor's cart into Angelus and runs up the stairs, there's no physical way he could be waiting for her at the top of the stairs (even with vampiric speed). Joss Whedon has admitted in interviews that he put in this blatant continuity error as the situation is a staple of the horror genre (girl runs from villain, only to find him standing right in front of her).

School Hard - S2-E3

Plot hole: When Buffy and Sheila are walking out of school after the meeting with Snyder, Xander runs up to talk to them. He mentions during this scene how Buffy will be helping out at the Parent-Teacher Night, but he shouldn't know this - Buffy only just found out about it in the meeting with Snyder.

School Hard - S2-E3

Revealing mistake: When Spike is talking to the Anointed One for the first time, one of the henchmen tries to rush him from behind and Spike backhands him without even looking, but Spike's hand movements, the reaction of the henchman and the sound of the punch connecting on the audio track are all out of sync.

The Succubus (a.k.a. Chef's Mama) - S3-E3

Stupidity: When Kenny and Cartman are sitting on the bench, Cartman can't start the tape as he has fallen asleep. Kenny then tries to wake him up, but wouldn't it have made far more sense for him to just start it himself, as he is sitting right next to it? Cartman's job was to push the play button. It was not like he had to rewind, fast forward or even eject the tape, so all Kenny had to do was just simply push the play button and tape would have started to play.

One Tree Hill (2003)

What Is And What Should Never Be - S1-E20

Plot hole: How in the world could Keith be hired as a teacher? Not only is he a college drop-out with no teacher training, Dan mentions just an episode or two earlier how Keith is incapable of using modern machinery (specifically saying how every high school graduate will be more qualified than Keith).

What Is And What Should Never Be - S1-E20

Continuity mistake: Haley's lipstick gets noticeably thicker and thinner between different shots when she's talking to Nathan (when she drops over some groceries).

The Succubus (a.k.a. Chef's Mama) - S3-E3

Other mistake: Cartman says, "What? What happened you guys? Is Kenny okay?" The bandages only covered his eyes, not his ears, so even if he's blinded by his bandages, he still should have been able to hear Stan shouting, "Oh my God! She killed Kenny!" and Kyle's reply "You bastard!"

The Succubus (a.k.a. Chef's Mama) - S3-E3

Continuity mistake: Kenny's head is completely smashed when he is attacked in the church during Chef's and the succubus' wedding, but it is whole again when Cartman brings it to the optometrist to get an eye transplant.

Shay

The Wish - S3-E9

Continuity mistake: Anyanka's demon face in "The Wish" is very different to her demon face any of the other times we see her ("Entropy", "Selfless" etc).
