Plot hole: When the professor put Travis' credits on the film he left in place of the real thing, some of the differences between film and credits would be very noticeable. For example, Sandra would be listed as having acted in the film, when she obviously wouldn't have been in the new film.

Factual error: When the girl with no kidney is decapitated, there's not nearly enough glass left in the window to take her head clean off while leaving it in the fixture.

Plot hole: The black guy who is murdered in the Tunnel of Death is hit in the head with a pickaxe. The coroner should have discovered the wound from this, which would have sparked a proper investigation into the deaths, which may have led to the killer being caught earlier.

School Hard - S2-E3

Plot hole: Willow and Cordy disappear during a vampire attack. We later see that they spent the entire night hiding out in a closet, but wouldn't Buffy have gone looking for them once the vampires were gone? She knew they had run off somewhere (she mentions it to Giles when she gets a stake from the library). Instead she just goes home, leaving them still missing as far as anyone knows. (00:41:25)

Continuity mistake: In the episode 'TOW Phoebe Runs' when Rachel runs into the horse at the end there is a shot of the horse looking round at her, when the shot changes angle to show Rachel picking herself up you can see that the horse is still looking straight ahead. (00:18:35)
