Unveiled - S3-E18

Plot hole: Vaughan discovers Lauren is a mole when he goes through her briefcase and discovers the wig, gun and fake passport she used in Germany a few hours before - surely even the worst secret agent in the world would have enough sense to get rid of such incriminating evidence as soon as possible, and not carry it back to the USA (sneaking it onto a plane, presumably) and leaving it in the room where the man you're lying to sleeps. Lauren is a highly skilled agent, if the Covenant selected her for a mission this important, so it's out of character (without an explanation) for her to be so sloppy.

Suspicious Minds - S1-E9

Plot hole: Mrs. Huber died at the end of the previous episode, "Guilty", so why is she signed up (with dress reserved and everything) for a fundraising idea Gabrielle only had in the teaser of this episode?

Suspicious Minds - S1-E9

Plot hole: Several days must have passed between the teaser of this episode and the rest of it, since Gabrielle has had time to make up (very professional-looking) cards and posters, gotten several dozen volunteers and reserved dresses for many of them, yet Paul has only just gotten around to disposing of Mrs. Huber's body and trying to wash the blood out of his shirt. Even if we assume the teaser takes place right after the end of the last episode (when Paul murdered her), that's still several days of keeping a rotting corpse and enough evidence to put him away for life around his house.

School Hard - S2-E3

Plot hole: When Buffy and Sheila are walking out of school after the meeting with Snyder, Xander runs up to talk to them. He mentions during this scene how Buffy will be helping out at the Parent-Teacher Night, but he shouldn't know this - Buffy only just found out about it in the meeting with Snyder.

What Is And What Should Never Be - S1-E20

Plot hole: How in the world could Keith be hired as a teacher? Not only is he a college drop-out with no teacher training, Dan mentions just an episode or two earlier how Keith is incapable of using modern machinery (specifically saying how every high school graduate will be more qualified than Keith).

Get It Done - S7-E15

Plot hole: When Dawn and Buffy discover Chloe hanging in the bathroom, Dawn gives a little gasp but nothing more. But, seconds later, some Potentials run from across the hall saying "What is it? We heard -" before they see Chloe. While it suits the next scene to have them there, there wouldn't have been any sounds loud enough for them to have heard from across the hall through a closed door.

Plot hole: What exactly was Laurie Strode's plan in the teaser, when she strung Michael up over the edge of the roof. From the way she starts cutting the rope it looks like she plans to drop him, but she's seen him survive a 2-storey fall before, in the original Halloween, not to mention multiple stab wounds in H20. A fall off the roof into bushes would barely phase Michael.

Halloween - S2-E6

Plot hole: Giles tells Willow to leave him while he extracts information from Ethan on how to break the costume spell. By the time Giles smashes the statue Willow has already found the others, who had left the house where she thought they were and are hiding in a completely different area of the town. No matter how long it took Giles to beat the information out of Ethan, there is no way Willow could have gotten back to the house, realised they were gone and tracked them down before the spell was broken. As a ghost, it is possible she could have happened across them even though they were hiding, but seems unlikely.

And Maggie Makes Three - S6-E13

Plot hole: When Homer is seen at work in the flashbacks (before Maggie was born), he is at his usual control panel in sector 7-G, but he only began working here when Burns made him safety supervisor in season 1, "Homer's Odyssey". Before Maggie was born he should have been working at the conveyor belt with the carbon rods, where we saw him in the first couple of episodes.

Five By Five - S1-E18

Plot hole: During the scene where Angel tries to work out where Wesley is being held hostage we learn that the man Faith beat up at the start is still alive in the hospital and that it's his apartment she's staying in. Angel bursts into the apartment to save the day, but that's not possible without an invitation, as the owner is alive, whether or not he's actually there. (00:34:00 - 00:36:40)

Plot hole: When George gets into the lift at her office, her reflection is very obvious on the sliding doors, but we were told back in the pilot that Reapers have a different visage to the outside world that shows up in reflections, on camera etc.

Phase One - S2-E13

Plot hole: In 'Phase One' Sydney tells Will during the day to "Get Francie and get out of town" because she realises there's someone after her. Later that night Francie is shot at her restaurant and replaced by Allison (the clone), but Will was supposed to have taken her away already. There's no question of him not finding her - the restaurant would be the first place he'd look for her.

Inca Mummy Girl - S2-E4

Plot hole: When Ampata is first released from her imprisonment, why didn't she break the seal then? She knew the only way to re-mummify her was to put the seal back together, so why not shatter it completely instead of leaving it in large fragments? (00:08:40)

Centennial Charmed - S5-E12

Plot hole: In 'Centennial Charmed' Paige orbs to another reality where she loses all her powers. Leo tells her the this is because she was never called as a witch in this world, but Paige had most of her powers before she was called because of her whitelighter blood, so she should retain these. Leo says that Paige has assumed the life of this reality's Paige, but then adds "...or at least the life she would have lived if she hadn't been killed". If alternate Paige had lived she would have had Whitelighter powers, even if she didn't know about them, so our Paige should also have them. (00:28:00)

Plot hole: How could the journalist kid have taken the photos of Lucas in the science lab drinking the weird liquid? Even Lucas didn't know where he was going until he got the phone call 20 seconds earlier, so how could his friend have been waiting outside the window with a camera?

Graduation Day (2) - S3-E22

Plot hole: In 'Graduation Day pt 2' the Mayor has his vampires stand at the bottom of the steps leading out of the school to stop any escaping students. The thing is, all the parents managed to escape out the side exits. Since the Mayor was expecting all the students to try and run too, why didn't he have these exits covered? (00:33:15)

Plot hole: How did Brenda get from the woods to her car and onto the road where Natalie was in the minute or so from the time she falls in the woods to the time she runs the janitor off the road?

What's My Line? (1) - S2-E9

Plot hole: If Spike and his gang only take a few hours to break through the door to Ford's club in 'Lie To Me' (which we are told is six feet of solid steel), why can't Angel break through a thin wire door that visibly shakes every time he hits it, given the same amount of time? (00:38:05)

Plot hole: When the professor put Travis' credits on the film he left in place of the real thing, some of the differences between film and credits would be very noticeable. For example, Sandra would be listed as having acted in the film, when she obviously wouldn't have been in the new film.
