Visible crew/equipment: When Indy enters the dungeon, he is using a match to lighten the writing on the wall. You can see the shadow of his whole arm which shouldn't be visible if that match was the light source. (00:53:15)
Audio problem: When Willie is trying to find the "Fulcrum Release Lever" she screams when Indy grabs her hand, but if you watch her mouth (just visible through her hair) it doesn't open. (00:58:20)
Revealing mistake: When Willie runs into the death pit after pulling the release lever, when she drops the lantern Indy is not Harrison Ford but a stunt double. (00:59:30)
Continuity mistake: Before the first guy is lowered into the pit, the leader of the group is taking out his heart. Before he takes the heart out it is his real body, but when he is taking it out it is blatantly a model, as it doesn't have the same amount of hair and is a different colour. (01:00:10)
Continuity mistake: When Willie is put into the cage, she has many necklaces around her neck. However, a few seconds later when Mola Ram walks up to her, all of the necklaces have completely vanished with no sign of anyone actually removing them before. (01:15:25)
Continuity mistake: During Willie's sacrifice, Chattar Lal appears suddenly grabbing his necklace and smiling between shots. (01:18:08)
Visible crew/equipment: After Willie spits in Indy's face it cuts to Short Round running through the mine, and just as Shorty turns his head toward the camera behind him, we can catch a glimpse of something blue which does not belong in this dark dreary environment, at the right side of the screen. There is a tall spotlight and two crew members, one is wearing a solid blue shirt and a cap, and the other a blue/white striped shirt with white pants. (01:21:25)
Continuity mistake: In the shot before Chattar Lal's hand and knife appear, Indy turns his head back. In the shot of the knife, he now looks at the wheel. (01:25:12)
Visible crew/equipment: When Indy appears at the entrance of the mining caves to free the enslaved children, he punches out one of the guards and he goes flying in the air and across the gravel. You can see two suspension wires attached to the actor to drag him back through the dirt. (01:27:45)
Continuity mistake: The shot of Pat Roach appearing at the slaves' caves, ready to fight Indy, actually seems to be a shot from the scene when Indy and Shorty are whipped by Pat and the Maharajah. This can be spotted by the tunnel and candles behind Pat. (01:28:39)
Other mistake: When the Thuggee throws the sledge hammer over his shoulder, notice that the man whose head the hammer lands on closes his eyes and leans his head slightly forward before the hammer hits him, as though he was already expecting the hammer to land on his head. (01:28:40)
Continuity mistake: On the mine conveyor, the guard pins Indy to the belt with his foot and his head toward the grinder. When Short Round tackles the Maharajah, we can see the guard on the conveyor, but he's facing the wrong way, both his feet are on the belt, and Indy is nowhere in sight. (01:31:30)
Continuity mistake: When Indy is fighting on the conveyor belt, he is about to be run over a couple of times, but in the next shot he is further away. (01:31:40)
Continuity mistake: When Short Round is dangling between the two carts, just before the cart tracks split, he goes from being held by one arm to suddenly being held by two. (01:33:25)
Continuity mistake: During the escape from the mine, Indy has to slide along on a rope to catch his companion's cart. Whilst hanging from the rope the shot switches from in front to behind a number of times. Depending on the shot, Indy switches from barely hanging on to having the rope grasped tightly next to him. (01:34:45)
Continuity mistake: When Indy kicks the Thuggee rifleman off the scaffolding, you can see in the background that Mola Ram is about to reach the scaffolding. In the next shot, he is no longer there. (01:34:54)
Revealing mistake: During the mine chase, after Indy cries to Shorty "Let it go, our only chance!" the wig of the doll replacing Willie flies away. (01:35:59)
Continuity mistake: During the 8 minute race on the mine carts through the mine. The group passes several bottomless cliffs, several huge holes, and even a hole of lava about 200 feet long. Yet when the water is released, it gets to them in about 30 seconds, and shows no sign of stopping when they outrun it. (01:36:00 - 01:39:30)
Revealing mistake: During the chase with the carts in the mine, in some of the shots, you can see that figures who are supposed to be Indy, Willie and Short Round are dummies. Indy's arm especially belongs to a mannequin in the shot where the cart almost derails. (01:36:00)
Audio problem: While they're in the tunnels, Willie's face appears to be normal while there's a scream of her in the audio background. (01:36:05)
Suggested correction: This takes place in milliseconds and there's no way to see it unless you pause the image or play it in slo-mo. It should be under trivia.

Right now I have it playing on Netflix, and there is no slow motion. The striped blue shirt is noticeable as it plays without any slo-mo. If something catches my eye onscreen, it is fair game to go back and rewatch what was noticed in the first place. And in order for me to confirm what I find, I do pause, and may take screenshots. By the way, I've seen this movie countless times over the years, and it was just a matter of catching that glimpse of the color blue because it popped out against the dreary surroundings. I'll leave it up to Jon whether to move it to trivia or not.
