Visible crew/equipment: In Riots, Drills and the Devil Part 2, when LJ's mom turns off the TV, the camera man's image can be seen in the glass next to the television. (00:18:20)
Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all. Visible crew/equipment: When Rimmer is being questioned by The Inquisitor, a full boom mic and arm can be seen rising out of shot by the three candles on the right of the screen. (00:08:35)

Visible crew/equipment: During a pan-across of Quark's bar after it has been emptied out, the shadow of the camera is visible for a moment in the bottom right corner of the screen. (00:40:35)
The Night Dr. Loveless Died - S3-E4
Visible crew/equipment: The arm of the actual carriage driver is visible poking through the back of the carriage, slapping the horse and signaling it when to stop/proceed with a second set of reins, showing the actor in the carriage was not controlling the horse at all.
Visible crew/equipment: In the opening scene, when Inspector Blake informs Stan and Arthur that the 10:45 bus has arrived, the camera pans over to the door of the bus where another cameraman's shadow can be seen on the side of the bus.
Mr. Monk Is At Your Service - S5-E12
Visible crew/equipment: In the scene where Natalie arrives to the dinner party "pregnant" and Mr. Monk is going to "help her with her coat" there is a female crew member on the other side of the sliding door (to the left of the door). (00:31:25)
Visible crew/equipment: At about 30 minutes into the episode, right after Cal says: "if Jeffrey knows about priox, he's going to jail. But I'm gonna be damn sure before I send him there", you can see a hand in the top right corner of the screen.
Visible crew/equipment: At the end of the episode, after Cook's father has been pushed off the boat, there should be only 4 people left on the boat (J, Freddie, Cook and Effie). However there is a camera man sitting behind Freddie inside the opening to the hull (you can see their arm/hand and camera lens).
Coming Home - S4-E1
Visible crew/equipment: In the scene where Joey and Pacey have arrived back at Capeside, and are getting things ready to get off the "True Love," in one shot you can see the microphone clipped to the back of Joey's shorts.
Visible crew/equipment: When they are at the crime scene, you can plainly see the camera and camera man reflected in Special Agent Lundy's sunglasses several times. When it is a close up shot, it is even more obvious.
Visible crew/equipment: After taking the enzyme, Mckay is talking to himself as he dials the gate. When he goes to push the centre button, an arm moves on the opposite side of the DHD. (00:09:35)
Visible crew/equipment: When Michael breaks into the car, after he drives off you can see a camera standing in the back seat in the car, and it's pretty clear it's a stunt man driving. (Helps to pause at 18:07.) (00:18:05)
The Crossing - S1-E2
Visible crew/equipment: After Hank Dale tells Steve Pinkus that he wants to know "How it feels to get away with murder," Hank throws a punch, and in the next wide shot we can see the second cameraman's arms holding an additional camera, presumably to film the close-up, at the right side of the screen. The Lame Duck Congress - S2-E6
Visible crew/equipment: At the end of the episode there is a camera visible in the oval office. After President Bartlet and Toby leave the office an attendant enters to turn off the lamps in the oval office. The scene widens as the lamps are turned off such that the audience can see the entire darkened room. The production equipment, a camera on tripod, and even a crewmember's leg are visible between the penultimate and final lamps being turned off. (00:41:50)
Visible crew/equipment: As Peter Sam passes Skarloey and Rheneas, a camera shadow is seen.
Visible crew/equipment: At the end of the episode, when Alicia is listening to the audio recording on her laptop in bed, you can see the hand of a crewmember on the corner of the white cupboard at the left side of the screen. (00:42:45)
