Visible crew/equipment: At the police station, when Florida is begging James to borrow money from the loan shark, she and James stand up; Florida is on the left, James the right. At the top of Florida's head behind her, you can see the microphone dip into view overhead, and it moves quickly out of the frame. Visible crew/equipment: While Phil and Claire are warming up for their race, Alex and Luke are getting dropped off. When you see them walking towards Phil and Claire you can see a crew member with equipment behind the bushes at the right side of the screen. (00:17:05)
Nightmares - S3-E7
Visible crew/equipment: While Jaime talks to Malcolm on the street and takes his picture it's being filmed from two camera angles. When Malcolm shoves Jaime and he falls to the ground we can see the second camera's tripod leg at the right side of the screen, and as Jaime gets up we can see the second camera as well.
Winterfell - S8-E1
Visible crew/equipment: After Sansa tells young Ned Umber to hurry back to Last Hearth and bring his people to Winterfell, when Jon tells Maester Wolkan to send ravens to the Night's Watch, in the next shot of Wolkan the chalk T-mark can be seen on the stone floor in front of his feet (it's not the stone's edges or cracks). Visible crew/equipment: At the end of the episode Saul is sitting in his office and you can see the shadow from a crew member in the windows. The member even moves. (00:50:41)
Visible crew/equipment: Fiona returns to work and tells her boss about the windshield. The camera backs up into his office. On the left side of the screen you can see the shadow of the camera operator.
The Case of the Lucky Loser - S2-E2
Visible crew/equipment: After leaving her husband at train station, Harriet takes a cab to meet a man at a cabin. After a few mins, she takes the man's car to go home. As she drives away, there's a reflection of a man, in suit with glasses (possibly the director) standing offscreen. (00:05:20)
Visible crew/equipment: [Unsure episode] When Judy proceeds to flip K.C. over, you can see a boom mike at the top of the screen, which K.C. narrowly misses.

Visible crew/equipment: When Barney falls over in the truck, as he is lying on the floor you can see his mic on the back of his trousers and a wire connected to it. (00:16:35)

Visible crew/equipment: In the petrol station, when the four are gathered around Simon, just before his phone goes off and Will asks, 'is that Leona Lewis?' part of a grey boom mic slips into the top of the shot for a second. (00:07:10)

Visible crew/equipment: During Flint's observation of the pow-wow, tire tracks from the camera truck are visible in the background.
Visible crew/equipment: Before the church scene, Mr. Bean drives into the car-park and knocks the blue car off the road. Look at the car closest to the camera, as Mr. Bean reverses, you can see a person and a crane above him reflected in the window. Also, in the blue car's window closest to the camera, you can see dolly tracks. (00:14:15)

Visible crew/equipment: When the guys are talking to Ari via cell phone from their black SUV, if you look at the black area between the front and back windows, you can see the reflection of the camera filming The Scene from outside the vehicle.
Episode #1.6 - S1-E6
Visible crew/equipment: When Lady Catherine de Bourgh and Lizzie are by the carriage (right after Lady Catherine de Bourgh questions whether Lizzie is engaged to Mr. Darcy), there's a camera or some sort of device visible. (00:36:40)
Suggested correction: The device being referred to looks like a lamp, which is part of the carriage. Therefore, it is not a mistake.
Blood Money - S10-E8
Visible crew/equipment: During the court case when Hamilton Stewart is taking the stand and answering to McCoy you can see the white screen in the reflection of his glasses very clearly.
Woman Seeking Dead Husband - Smokers Okay, No Pets - S1-E4
Visible crew/equipment: When Shawn and Gus gets out of the car at the Mausoleum Parking, you can see the camera man reflected in the mirror inside the car.
Dukes Meet Cale Yarborough - S2-E7
Visible crew/equipment: When the General Lee makes a great jump (in fact the jump is used in the opening credits of the show starting in season 2), you can see a camera hooked to the bottom of the car on the passenger's side.
Visible crew/equipment: When Kel gets up to get more water you can see one of the cameras.
