Audio problem: In the motel, Woody Woodpecker's laugh does not match up to the motion on the screen.
The Last of the Cybernauts ...?? - S1-E3
Audio problem: During the opening scene when the cast are gathered around the piano singing "Happy Birthday" to John Steed, his voice is audible as a part of the singing from the start, but he doesn't start singing until the second line.
Audio problem: In the last scene of the episode Sheridan says to Mr. Sebastian "The morning after the last of a string of murders in the East End." His lips however, say West End instead of East End. The script and initial broadcast used the words East End, but after it was pointed out that this was factually incorrect, the line was dubbed over for all subsequent broadcasts and releases. Audio problem: During the opening credits no one moves their mouth when they notice Superman and say "it's a bird...it's a plane..."
The Night of the Cut-Throats - S3-E11
Audio problem: Arte's hand and finger movements on the saloon piano don't even begin to match the notes actually being played. (00:07:30)

Audio problem: At the end of the episode when Reddington goes to see Natalie Luca (behind glass), he tells her that the woman she killed at the massage parlour was an associate of his. When Natalie says "She said if I didn't kill him, she would tell Hawthorne where to find us", her lips don't match what she's saying in the reflection of the glass.
Audio problem: When Barney and Willy are digging their tunnel toward the prison basement, Barney's voice and lip movements are badly out of sync. (00:31:05)
The Two-Hundred-Mile-an-Hour Fastball - S2-E1
Audio problem: When Deborah gets off the phone, she tells Ralph that if they make the deal they can announce it on the Mike Douglas Show, but the problem is each time we hear Ralph say the name "Mike Douglas" we see his mouth actually say "Merv Griffin." (00:13:30)
Audio problem: Near the end Robin's men all push barrels of oil over to to create a wall of fire, trapping the Sheriff and his guards inside the castle. When Robin picks up the flaming arrow he shoots it at the oil where it sticks firmly in the ground, but if you listen closely when the arrow hits the floor there is the sound of the arrow bouncing against stone - it couldn't make that noise if it hit the ground directly and stuck there.
Audio problem: In the final scene Danny executes the man who killed his brother. Danny is armed with a snub nosed revolver. When he pulls the trigger, the sound of a shell casing bouncing on the floor is heard. A revolver does not eject empty shell casings.
Did You See the Sun Rise? - S3-E1
Audio problem: The noise of TC's helicopter powering down is Out of Sync with the speed of heli blades you can see in the reflection of the glass, and the effect on nearby vegetation. (00:09:40)
Yugi vs. Pegasus: Match of the Millennium - Part 3 - S1-E37
Audio problem: When Bakura tells Tristan to hand over Mokuba or else he'll suffer the same fate as the guards, he starts doing an evil chuckle, but his mouth is open and it doesn't move. (00:17:30)
Neogenic Nightmare Chapter 4: The Mutant Agenda - S2-E4
Audio problem: When Spider-Man is saying to Beast "I'm not sure I still am," Beast is the one mouthing the last three words in the line. (00:09:25)
Audio problem: Volkswagen Beetles had a very distinctive "putt-putt" engine noise. In every scene with the bad guy following Ryan and McBride in a VW Beetle, the sound effects dubbed in post-production belong to a much larger, standard American car, and not to a VW at all. (00:26:00)
