Factual error: During the scene where it shows people all over the world, just before the shuttles take off, it is daylight everywhere. It would actually be dark or near dark in parts of the world.
Revealing mistake: The kid in the wheelchair is playing a Game Gear with no game in it.
Continuity mistake: In the scene towards the end of the film where we see Slade's body fall, it falls onto a submerged rock just below the surface of the water and there is a splash of water. In the next shot Tarzan looks down from the cliff above and Slade's body is on a large dry rock, a little distant from where the body originally fell.
Factual error: In the first scene where Dr Strange is preparing for surgery he uses improper surgical gowning technique breaking aseptic protocols. Strange puts on a mask after washing his hands, contaminating his clean hands by bringing them in proximity to the non-sterile environment of his face. Surgical masks must be worn before the full washing of hands. Another error is that he inserted his hands all the way through his gown to don gloves. Hands must never leave the sleeves and gloves must be put on with the sleeves still covering.
Revealing mistake: In the final scene where Fonda and Holbrook are looking over the carrier at the crowds on the dock you can see that only the first rows of people are dressed in period clothes, the rest of the crowd are dressed as they would have been in 1976 when the film was made.
Continuity mistake: In the opening scenes where he gets his mustang back, he hits the guy who comes over the roof and busts the windshield, but in the very next shot the windshield is pristine. (00:07:45)
Continuity mistake: When Indy tries to grab the antidote vial from the table, in the closeup just as the vial falls off the table the camera follows the vial as it lands in the empty area to the left of Lao Che's chair where Kao Kan's chair should be, but it's not, and the rug's edge with wood floor beside Lao Che's chair though it shouldn't be. Then Indy hits Kao Kan who is again seated closely to Lao Che's left, as he should be, and when Kao Kan falls backward we see the rug edge is nowhere near Lao Che's chair, also as it should be. Continuity mistake: It was briefly stated in the second film that the T-Virus was created by Dr. Ashford to cure his daughter Angela and that the Red and White Queens were based on her. This movie contradicts that by suggesting that Dr. Marcus created the T-Virus to cure his daughter Alicia, and that the Red/White Queens were modeled after her. While the rest of the movie stays true to the continuity of the previous films, this rather obvious ret-con is still a major continuity gaff.
Revealing mistake: When The Mask is dancing with Tina and he turns her into a human tornado while spinning her around, none of the onlookers in the crowd show the slightest bit of amazement at the impossible feat that just occurred in front of them, indicating that the extras were not directed to react to the special effects.
Factual error: When Philby and Alexander are talking about Albert Einstein, Alexander mentions that Einstein is a patent clerk. The beginning of this movie takes place in the year 1899. Einstein was still in school and didn't become a patent clerk until 1902.
Revealing mistake: When the kids are burying Gus, his supposedly dead face flinches when they shovel dirt on him.
Factual error: It is roughly 120-135 dB inside of a C-130, especially one that hasn't been specifically modified. It is so loud that it is painful to be in the cargo hold without hearing protection (besides it being outside of military regulations) and you certainly couldn't have a normal conversation while it was in flight.
Continuity mistake: As the hand creature busts the shackles off Boris' legs in Lunar Max, you can see the bottoms of his pants over his metal prison boots. There are only a few folds and wrinkles over his left foot, and the pants over right foot are hardly wrinkled at all, or affected as the creature climbs up his right leg. But only a couple of seconds later, we see another shot of Boris' feet as the creature breaks off the metal sleeve of his arm, and suddenly his pants legs are very wrinkled. Boris was standing perfectly still the entire time. Boris' legs are also suddenly closer together. (00:03:35)
Factual error: Mel and Goldie board a ferry. A visible sign states 'Detroit-Racine Ferry'. Detroit is on the east side of the state of Michigan but Racine, Wisconsin is west of Michigan, across Lake Michigan. The only water route to get from Detroit to Racine would be to circumnavigate the entire state of Michigan.
