Plot hole: There are six metal bars mounted around the circumference of the water wheel's axle. In the wheel's interior, when Jack frees himself he lands on his feet, begins to run and promptly hits his head on one of those axle bars, then falls out. Moments later, when Jack returns and runs in the wheel, he is shorter (way more than a foot) than all six spinning axle bars, which would make that previous shot impossible, however humorous it is. (01:52:40 - 01:53:55)
Continuity mistake: In a scene towards the end of the movie where Bond and Vesper are lying on the beach, Bond has sand on his back through most of the scene. When they start to kiss, in the distant shot towards the end of the scene, he has no sand on his back at all. (01:57:10)
Factual error: When performing CPR on Elena, Robert doesn't tilt her head back. As a former firefighter, he would know one of the first steps of CPR is to tilt the victim's head back to clear the airway. His rhythm is also way off.
Revealing mistake: In the disco scene, the DJ is playing music, but the Pioneer decks and the DJ Mixer are not even turned on, as no lights are showing.
Plot hole: The snakes escape from the locked box stored in the cargo hold when a small explosive bolt detonates and opens the lid. Post 9/11 all luggage placed in the cargo hold of an airliner would be tested for explosives, and anything larger than half a match head would be detected. The chances of getting an explosive bolt on board - nil. None whatever. You can also think of it this way - if the bad guys could somehow smuggle explosives on board, why bother with such an elaborate and flawed plan as letting snakes loose on the plane? Why not just smuggle a bomb on board? Pan Am 103 - the so-called Lockerbie flight - was brought down with a bomb that fit inside a small cassette player.
Plot hole: Christopher Columbus speaks Italian while the Huns supposedly speak their native language. Shouldn't Octavius and his Romans speak Latin? (00:34:50)
Continuity mistake: At the end, during the fighting in the Cathedral, the last woman left is getting first cut by Steven Seagal at the right breast. A moment later, her cut is on her left side. The next moment it is on the right side again.
Continuity mistake: In the long crash scene at the end, the Perrier car loses the front passenger side tire in the initial flip, but in the next shot it is attached to the car again.
Factual error: As the two fast boats start heading up river towards the finale shootout, one boat has its navigation lights reversed. It has a red light on the starboard/right side, and a green light on the port/left side. It should be the other way around. (The lights on the other boat are correct.)
Plot hole: Where did Cross get the blood on his hands? The gloves were clean of blood after he looked at the teeth and removed the gloves, so where did it come from?
Factual error: During the big sacrifice scene an eclipse happens. An eclipse occurs when the moon passes between the earth and the sun. Later, apparently that night, the night mood is set by a shot of a full moon. The moon cannot be full until fourteen days after an eclipse, when it is directly opposite the sun.
Continuity mistake: When Leonidas, and presumably all the other Spartans, use their spears to clear their shields of arrows after the arrow shower from Persian archers, the tips of the arrows are still visibly left on all their shields. In the next shot, when the Spartans regroup for the Persian cavalry charge, the arrow tips are gone. (Granted, this is a highly stylized movie based on a comic book, but that does not change the fact that this is a continuity error.)
