Visible crew/equipment: After both siblings are carried out of the burning house, when Roy kneels down beside the boy and leans forward to set down the drug box, the bulging outline of Roy's mic transmitter is visible (till the end of the scene) at the front, left side of his shirt, and note that the actor even touches it, possibly to make sure it didn't move out of place.
Visible crew/equipment: At the refinery fire, after Roy gets into the ambulance with the patient Johnny shuts the back doors, and when the ambulance backs up the reflection of a crew member with paperwork (script?) is visible in the left ambulance window.

Visible crew/equipment: After Johnny rappels down alone, so he can inform the sheriff of the situation with the injured climbers, when he talks to the sheriff beside the squad car, the reflections of the camera/dolly/crane are visible on the chrome corner of the sheriff's car.
Continuity mistake: When Roy and Johnny are on the Coast Guard helicopter, responding to The Boat wreck with injuries off Catalina, there are a few interior shots of them that are flipped, while they're looking out the windows. The paramedic patches are on their left sleeves, but in the flipped shots the paramedic patches are on their right sleeves.
Visible crew/equipment: At the skating rink, when the guys start walking on the ice and go to the two clowns, the outlines of the mic transmitters and wires are visible on Johnny's shirt, as well as on the back of the blonde skater's green outfit. Then, when the ambulance attendants are walking on the ice, the outline of the lead attendant's mic transmitter is visible on the front of his shirt.
Visible crew/equipment: After spending all night on the job, when the tired guys of 51 are at the basketball game, the outlines of the mic transmitters and wires are visible on the backs and sides of 51's jerseys, particularly when Roy gets injured. (00:36:00)
Other mistake: Station 51 responds to an "accident with injuries, " address 7526 Sunset, cross street, Abbott, to rescue two people stuck on scaffolding at the top of the tall building. This distinctive, mirrored glass building was also used this season in episode 6, "Rules of Order", with the address given as 3120 South Century, cross street, Burgess, for the rescue of two people hanging off the side of the tall structure. The actual building address is 500 190th Street, Gardena, CA.
Loose Ends - S6-E12
Visible crew/equipment: At the hospital, when Gage sells the picnic tickets to Dixie and Dr. Early, just as Dr. Brackett walks up to the nurses' station, both Brackett's and Johnny's T-marks are visible on the floor.

Visible crew/equipment: Right after rescuing the two injured climbers, when John and Roy are in the back of the pickup truck with them, just as the pickup drives off to follow Sheriff Bittner's squad car, we can see the reflection of crew and the camera rig on the Land Rover's windows.
Revealing mistake: At the site of the hang gliding accident, just after the glider falls, in the long shot when Johnny and Chet pull the unconscious victim higher on the cliff, the victim is supposed to be unconscious, yet we see him use his right leg to push himself higher up to help John and Chet pull him up.
Visible crew/equipment: At the gas station with the broken pump nozzle, when Roy and Johnny are tending to the woman with the cut on her head, the outlines of Roy's mic transmitter and wire are visible on the left side of his shirt, above his belt. Additionally, note that the office's left pane of glass has been removed to prevent the reflection of the camera facing it.
Continuity mistake: While Charlie is finishing up on the squad's engine, Captain Stanley hands Johnny the address in the canyon that has overgrown weeds, and when Johnny packs up the toolbox, Charlie holds out the roll of electrical tape and the combination wrench, with its open-end up. However, in the next shot the only thing Charlie holds out is the wrench, but with its box-end up.
Visible crew/equipment: While Johnny and Gage are in the press box with the cardiac arrest, when Brackett tells Johnny to administer the bicarb and epinephrine, the bulging outline of the mic transmitter is visible on the left side of Johnny's shirt.
Visible crew/equipment: When Roy and Johnny are stuck in the elevator with the cardiac patient, the outline of the lav mic transmitter is visible on the front, left side of Roy's shirt, above his belt.

Additionally, Dr. Early's mark (during The Exam) becomes visible when Dixie shifts her position, just as Dr. Bracket asks, "Any pain here, Roy?"
Visible crew/equipment: When Station 51 arrive at the movie studio, after the car's explosion Captain Stanley tells dispatch to respond another engine, mobile aid unit, and another ambulance. In the next shot from the interior of Engine 51, the cameraman and camera are visible in the passenger side mirror.
Loose Ends - S6-E12
Visible crew/equipment: When the guys bring Brackett to the hospital after his car accident, two white tape, and one black tape actors' T-marks are all visible on the floor in the ER.
Continuity mistake: When Station 51 is dispatched to 'the man down' at the hotel, Roy gets into the squad fully dressed (he was talking to Chet) and wearing his blue uniform shirt, but when they're in the apartment his shirt is gone and he's only wearing an undershirt under his jacket, though Johnny is wearing his uniform shirt.

Correct, it was J. Pat O'Malley...he also played the grandfather with his grandson when their rocket exploded, and also played "Old Bill" in the episode with Ann Prentiss, where Gage saves the little girl from the burning tree house, and her mother falls in love with him.
