Visible crew/equipment: When Gibbs and McGee are talking to the kid at the start there is a camera shot moving past an empty chair. In this shot a shadow can be seen moving over the chair. (00:04:00)
Continuity mistake: When Maddie turns the photo album round, its position changes between shots. In the first shot she is holding it by the top of the album, as shown with a brown photo being away from her hands. Camera changes and now she is holding it by the bottom of the album and her hand is on the brown photo. (00:09:40)
Continuity mistake: We see Jenny talking to the former embassy worker. When doing so she has her hands out stretched in one shot and clasped in others. (00:25:30)
Revealing mistake: When Ducky opens the fridge and uncovers the body, the man's eyes twitch. (00:21:20)
Revealing mistake: In a close up of the iPod there is 1 song on the play list, playing at the 29 second mark. However, the icon to indicate that the iPod is on repeat is missing. Therefore, the iPod has only been playing for 29 seconds, despite the fact the crew left a good time before the team arrived. (00:07:20)
Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
