Audio problem: In the transporter room, as Kirk is talking to Sulu, the door opens, McCoy comes in, and the door shuts. Trouble is that the characteristic sound of the door is missing.

Audio problem: As Spock answers the call from the ship, his communicator doesn't make the noise when connecting.
Audio problem: After McCoy gives orders for equipment, the Communications officer calls for Kirk. As he goes to answer, Kirk flips open his communicator, but it doesn't make the familiar sound.
Audio problem: As McCoy scans the body, the scanner portion spins. Suddenly, it stops motion, but McCoy continues passing it over the body, and sounds continue to be heard.

When Kirk and Sulu enter the records room, they pick the lock. Later when they beam the officer back down, he enters the room without unlocking the door. The room should be locked since they beamed him down in the "past" erasing their having been on Earth and in the records room.
Gene Roddenberry created the transporter as an easier (and cheaper) way of getting Enterprise crew members onto a planet's surface, rather than landing the ship on the planet.
