Other mistake: As the Romulan commander goes to answer the intercom, the switch she touches to open the link is already on, probably left that way from a previous take.
Factual error: Mr. Spock states that the asteroid is almost as large as the Earth's Moon. Such big bodies must be predominantly spherical due to their own gravity. However, the asteroid is very elongated, with a very irregularly shaped surface. (00:06:40)
Continuity mistake: When Miramanee and her companion first see Kirk at the obelisk, the two women are standing, holding their fruit baskets, just inches from each other. In the very next, reverse angle shot, they're suddenly several feet apart. (00:09:10)
Factual error: When Kirk is given the medicine badge, it's a stretchy elastic/Spandex headband. This is supposedly a completely pre-industrial culture paralleling the early Native American tribes. They have no fabric, no yarn, no spinning wheels - only hides and animal hair, neither of which can stretch a la Spandex. (00:14:40)
Continuity mistake: When Spock orders the phasers to fire, they shoot outward from the ship at an angle from each other, but when they hit the asteroid they converge at a single point. (00:24:00)
Continuity mistake: When the Enterprise is trying to push away the asteroid using the deflector, a quick shot of Uhura is shown from stock footage of "And the Children Shall Lead," but she is not in the rest of the episode.
Plot hole: If diverting the asteroid was so important, what was the crew doing on the planet to begin with?

Continuity mistake: The stardates of Starnes' entries are 5025.3, 5032.4, and 5038.3 But Kirk gives the stardate of when they arrive at the beginning of the episode as 5029.5, so apparently Starnes made two entries after he died.

Other mistake: At the end of the episode, Kirk commands the Enterprise to Star Base 4. Earlier in the episode, there was an unsuccessful attempt to beam up two crewmen left on the planet Triacus. They evidently remain there to this day.

Audio problem: When Spock asks Kirk, "Why are they bothering Starfleet?", there is a loud buzzing noise twice as they are focused on Kirk's face.

Continuity mistake: When Gorgon the Angel is on the bridge with the children, the positions of the children are different depending if the shot is from the rear of the children or the front.

Plot hole: When the Medusan Ambassador Kollos and Dr. Jones materialize, the latter is wearing a protective visor and Mr. Spock can see that. Spock then mistakenly addresses Dr. Jones as Ambassador Kollos. Spock's suddenly illogical mistake seems odd: why should the Ambassador need to wear a visor? Shortly after, why do both Jones and Spock wear visors while carrying the Ambassador's still closed gasket to his cabin? (00:01:45)

Visible crew/equipment: Spock and Dr. Jones carry Koldos into his quarters, and when Spock walks over to the wall communicator to contact the bridge, the camera casts a shadow on the wall as it moves closer to Spock. (00:05:20)

Continuity mistake: At the dinner for Miranda, Kirk drains his glass. No one refills it, but it's nearly full again a few shots later. (00:08:25 - 00:10:35)
Suggested correction: Kirk's glass is not empty at 8:25, it is the top of the decanter in front of his chest that looks like an empty glass in the shot. Later Kirk empties his glass during toasts and refills his own glass as he watches Marvik leave the room.

Visible crew/equipment: At the dinner, when Miranda stands up to leave, there's a visible white tape mark on the floor near her feet. Then, when McCoy makes his exit Kirk reaches for the rose, and there's a better view of the white tape mark under the table. (00:11:50 - 00:13:45)

Revealing mistake: During the fight in engineering, Marvick's kick, throwing one of the engineering crewmen violently back against the wall, misses by an obvious mile. (00:20:10)

Visible crew/equipment: When Kirk turns to follow Miranda across the arboretum, the camera shadow is briefly visible on the wall at the lower left of the screen. (00:29:50)

Deliberate mistake: While Miranda is with Kollos, McCoy tells Kirk that he didn't reveal her secret because he respects her privacy, then when Miranda steps out of the room and concedes to obey, the next closeup of Kirk is a flipped shot. (00:34:05)
