Revealing mistake: Throughout the episode, the difference between the actor's real ear and the prosthetic point is very obvious. Note the scene at the beginning in the conversation between Spock and Uhura; the difference in Spock's makeup from the real and fake ear is very obvious.

Plot hole: The salt creature needs salt, but there is no need for the creature to kill anybody, just have them deliver a large shipment of salt the next time some ship come to visit the planet.
Suggested correction: As shown several times, the creature is acting on craving and impulse. It's able to not attack Dr. Crater because they have a mutual relationship, as well as he provides salt for her from their supplies. But their supplies are running low, as he even stated they did have 25 pounds of salt but displays a jar with barely any left in it. It is reasonable to assume that he has had to start rationing the supply of salt to her to keep her till they could get another shipment. They were not getting the salt right off after Kirk arrived, and so the creature could not resist the urge to suck salt it needed out of the crewman when they were together alone, reason and logic being clouded by its desire and feral cravings.
Revealing mistake: When McCoy is examining Charlie in sickbay, several shots of the diagnostic panel show a clear reflection of Charlie's upright profile. But Charlie isn't sitting up during the exam - he's lying flat on the table. The close-ups of the panel were shot while actor Robert Walker Jr. was either sitting or standing nearby. (00:04:10)

Visible crew/equipment: After McCoy examines Charlie and they talk about how Charlie learned to talk by just listening to the ship's tapes, when Charlie stands up the two actors' marks can be seen on the floor - the short one for Charlie and the longer one for McCoy, where they both will stand momentarily. (00:04:56)
Continuity mistake: After Bones finishes examining Charlie, they move to the door and Charlie begins to put his jacket back on. In the side shot, he gets it on and begins to adjust it around his shoulders when it cuts to a frontal view. Suddenly his arms are relaxed like he's already had the jacket on for a few seconds and in a comfortable position. (00:05:18)

Revealing mistake: As Charlie watches the pole lowering into a square hole in the deck, the wood grain of the set piece is visible. The wood has been painted gray/silver to look like metal. (00:06:06)

Visible crew/equipment: When Charlie catches up with Rand and gives her the present, the microphone can be seen moving around at the bottom of the screen in the shots with both of them. (00:06:48)
Other mistake: Spock and McCoy have a brief argument over whether Charlie could have survived on his own all those years. Because they're talking at the same time, Spock saying "Doctor" as McCoy says "Mr. Spock," the closed captioner apparently misunderstood McCoy's line. The caption has him calling Spock "Dr. Spock." (00:09:15)

Continuity mistake: Just before Uhura begins to hum with Spock's playing of his instrument, she is leaning over a table where they are playing cards. She has one hand on her cheek and then brings the other up to her face as well. When it cuts to a side angle, she's suddenly bringing her hand up to her face again in a repeated action. (00:09:47)
Audio problem: When Uhura apologizes to Spock for interrupting him playing his instrument, her word is cut off mid speak between cuts. She says "I'm sorr-." But it's close enough it easily got past the editor or the editor cut her word off too soon not realizing. (00:09:59)

Continuity mistake: Uhura starts to sing and walk around Spock as he plays. In Spock's close ups, the wall behind him is pink and green, but in the further shots the walls are a blue color with nothing behind him that could have been making the green and pink colors in the close ups. (00:10:50)

Continuity mistake: When Capt. Kirk and Charlie enter the turbolift heading for the bridge, Kirk is wearing one style of tunic and when the turbolift arrives on the bridge, Kirk is wearing a completely different tunic. (00:15:14)

Continuity mistake: When Kirk begins to question Charlie about the melted chess pieces, there is a close up of Kirk showing light cast down him him with his chest void of any shadows. When cut to a wide angle, he suddenly has shadows on his chest from lighting changes. (00:22:36)
