Other mistake: The only way to beam up to the ship is to repeat the chess counter move after ASKING to be beamed aboard. After the two Captain Kirks fight ends with Spock stunning Garth with the phaser, Spock contacts the Enterprise. After saying "Spock to Enterprise, and hearing Scotty say back to him "Scott here", Scotty says" Queen to Queen's level 3" without Spock requesting to be beamed aboard or even waiting to see what Spock wanted in the first place. Continuity mistake: After Garth attempts to convince Mr Scott that he is Captain Kirk and to beam him up, while he is banging his fists on the floor in anger he transforms back into himself and the fly like ring on his finger breaks and slides out of shot. It's quite hard to see but it does. The next time we see Garth's hand his ring is intact yet he has not transformed himself again to repair it.
Continuity mistake: When Scotty thwarts Garth, disguised as Kirk, attempts to beam to the Enterprise using the Queen-to-Queen level three code, Kirk throws a tantrum and accidentally loosens one of his controls during his rant right before he turns back to Garth.

Other mistake: At the end of the episode, as Lokai is running through the ship, his hands are empty. However, in one closeup shot he is holding onto a rope with his left hand. The rope is gone in the following shot. Presumably this rope was used to tether him to the camera as he ran so that he could maintain distance from the camera.

Revealing mistake: As Kirk turns all the way around to look at the newly arrived Bele, the shadows of the stage wiring is easily spotted at the top of the screen.

Other mistake: When preparing to go around again, Sulu and Checkov start working their controls. When there is a close up of Sulu's hand, it's obviously a re-used shot of Kirk's hand starting the 30 second count down timer on the Constellation in "The Doomsday Machine" (S2 E6).

Revealing mistake: After Kirk shuts down the self destruct, he asks Spock if the ship is heading for Arianas. At one point in the dialog, you can see the display for the self destruct sequence is still on the screen, just not lit up.

Revealing mistake: When the camera is close up on Beal's face, you can see a bit of real flesh color where the black and the white greasepaint used didn't quite match up.
Continuity mistake: When Kirk beams down to the planet, he selects the front transporter pad next to the steps, and closest to the door. When he materialises in the replica transporter room, he is on the same pad. When the scene cuts to a view of the empty transporter room, Kirk is no longer on the front pad. Continuity mistake: When Kirk splits up the landing party, he asks McCoy to further inspect the vegetation. In the two shot, McCoy isn't wearing his tricorder, but in close-up, it's there, strapped over his left shoulder. Cut back to the two shot, though, and it's gone again. (00:10:20)
Continuity mistake: When Losira appears to D'Amato, he looks up from his tricorder, startled. The shot then changes angles, and D'Amato looks up at her all over again. (00:11:55)
Continuity mistake: When Mr. Scott asks Watkins to check the bypass valve, Watkins is facing the engineering boards with both hands on the panel. When we cut to a different angle, however, he's suddenly jumped to a position facing Scott with only one hand on the panel. (00:18:35)
Continuity mistake: When Losira comes for Kirk, he's holding Sulu's tricorder with both hands. When the angle changes to include her, he has it in one hand with his right hand at his side. Cut back to a three shot of Sulu, Kirk and McCoy, and Kirk has the tricorder in both hands again. (00:36:55)
Other mistake: In this episode, when Kirk's phaser was on overload, it made a small explosion, but in "The Conscience of the King", a phaser on overload would destroy 2-3 decks of the Enterprise.
Character mistake: Watkins is asked to check the bypass valve, there the woman appears, asked about the panel, it's obvious she's not part of the crew. Instead of calling security or Mr Scott he starts to describe the panel, which I'm assuming would be classified information.
