Continuity mistake: When Mark and Elizabeth are coming round the corner to enter the hospital, Mark says, "Remind me again why you invited her to stay with you", and he has one arm around her and in the other hand he's holding a newspaper. As the shot changes when they go round the corner, his arm is no longer around her and both hands are on the newspaper. There was no time for the switch.
Continuity mistake: When Romano is operating on the heart patient Valerie, Lucy has an instrument inside the patient. The shot then changes so that we can see her face and as she says, "Dr. Romano," there is no longer anything in her hand and she is holding it up waiting for a nurse to pass her an instrument, which she takes, but then the shot goes behind her - she now has a different instrument in her hand, which is in a different position.
Continuity mistake: When Dr. Lawrence is tending to the patient with the stabbed wrist, the blood splatter on his lapel goes from bright red to brownish and back to red within a few shots.
Continuity mistake: After Carter pulls the IV out of the patient who was allergic to sulfa, Malik asks, "Where's all the blood coming from?" and we see the patient's arm outstretched, bleeding from where the IV was. Carter says, "She must have pulled the IV out", and we see her arm folded up over her stomach - there was no time for the change in position.
Continuity mistake: Carol puts a chart on the counter and says, "I need 2 mgs of morphine for Mrs. Bragha", and Doctor Dave signs, with both arms on the counter when the camera is on Carol. But when the camera is on him, he has has one hand on the counter and one arm down by his side. This happens for a few shots.
Continuity mistake: When Mr. Perez pushes Carter into the window, Carter's stethoscope is around his neck with the earpieces on the right. At one point during the fight the stethoscope is around his neck with the earpieces on the left - and then returns to the other position.
Continuity mistake: When Carter goes into the lounge, he leans his crutches against the lockers and then moves his bag off his shoulder. One crutch falls to the floor, then the bag, then the other crutch. When the shot changes to show these items on the floor, the bag is lying neatly on top of the crutches - an impossibility due to the way they all fell.
Visible crew/equipment: When Dr. DeRaad asks Kerry if she knows Paul Sobriki she tells him to check the board, and as Kerry walks past the door we can see the reflection of a crouched crew member walking backwards, pulling a rolling microphone stand. (00:02:45)
Visible crew/equipment: When Luka and Mark are questioning Amira about the knife Sobriki had used, Kerry walks in then tells Amira to go upstairs to get security to open the personnel office, and the green T-mark taped to the floor is visible near Luka's feet. (00:16:45)
