Revealing mistake: When Susan and Chloe's mom comes to the hospital to visit, Chloe gives the baby to Susan to hold. If you look closely you can tell the baby isn't real but is actually a doll.
Continuity mistake: When Susan and Chloe are fighting in front of the desk, you can see 2 marks on Susan's right shoulder that look like blood with no obvious source from previous scenes. While Chloe is having her hand treated, Susan is standing outside the door and turns, and the marks have disappeared. (00:35:05 - 00:35:50)
Continuity mistake: Benton's stethoscope moves between shots as he talks to Hicks about failing to get allowances to harvest a patients organs for donation. As he comes out of the room the stethoscope lies over his shoulders, in the next shot of Benton it's hanging round his neck.
Continuity mistake: Doug introduces his girlfriend's son Jake to old girlfriend Linda who is holding her coat over her arm with her hands clasped together. As she says hello to Jake she puts her hand up to her hair, but from his angle we still see her hands together. This continues for the next couple of shots as they chat.
Continuity mistake: Carol is seen walking into the ER twice at the beginning of the episode.
Everything Old Is New Again - S1-E26
Continuity mistake: When people start to dance at the reception, there is a shot of Susan, Doug and Mark. Then the sax player approaches Susan and asks her to dance so she leaves and hits the dance floor with him. They show them dancing, then a few seconds later go back to the people standing by the table and Susan is next to Doug again. (00:42:00)
Visible crew/equipment: Dr. Kayson gets on the elevator, and Susan tries to get his evaluation about her patient with the mitral valve prolapse. Then, when Susan walks away from the elevator, as the rolling camera tracks to the right, its reflection is visible on the glass.
Visible crew/equipment: The second time Carter stands ready in the OR, Nurse Shirley draws a circle on the floor around Carter's feet, then tells him, "Don't move!" While Morgenstern and Benton continue their round of foreign capitals during the surgery, just as Carter adds, "Kinshasa," the boom mic pops up into view at the bottom of the screen. (00:38:50)

Visible crew/equipment: While Carol is at the admit desk, Linda returns to the ER, and as Linda says, "My Lexus just got snowplowed in; could I use the phone?" the reflection of the moving boom pole is visible on the partition.
Continuity mistake: While Mookie stands at the doorway of Trauma 1, where the patient's traumatic amputation is being treated, Lydia gets splattered with the man's blood and says, "Whoa! I got a pumper," but in the next shot that blood has suddenly vanished from her face and safety glasses.
Visible crew/equipment: Susan searches for a surgeon for Mr. Bozinsky's aortic aneurysm. When Bozinsky has coded, Bob cuts the man open, then does the cross-clamp procedure herself, and in a couple of Wendy's close-ups, the reflection of the boom microphone is visible on her safety glasses.
Audio problem: While Susan is with Mr. Bozinsky, Lydia takes Mrs. Bozinsky to the payphone. Then, when we hear Susan say, "Mr. Bozinsky, are you sure when I push here, this isn't hurting you?" Susan's mouth is not actually saying anything.
Visible crew/equipment: Right at the start, Hathaway wakes Ross and Carter from their naps, and as she says, "Carter, med students don't sleep at 4:00 in the afternoon," there's a visible boom pole/mic at the top left side of the screen, that gets pulled up out of sight when the lights turn on.
Visible crew/equipment: After Jerry shows Mark how to use the paging microphone at the admit desk, the camera dollies out just as everyone joins Mark for Carter's prank, and there's a director's chair, with a crew member (has a goatee/beard) standing behind it, that is visible at the left side of the screen.
Visible crew/equipment: While Jerry is watching TV news about the snowstorm, Mark asks him how the desktop paging microphone works. Then, as Jerry walks toward Mark, the reflection of the boom operator moving with the boom mic is visible on the glass partition.
Visible crew/equipment: While Doug is treating the animal-rights activist, Francis sees Tag plucking the turkey in the next room, and when he asks Doug to shut the blinds, the reflection of the moving boom microphone is visible on the window's glass.
Going Home - S1-E4
Visible crew/equipment: While Doug and Carol are talking outside, Carol says, "You think you had something to do with my swallowing a whole bunch of pills," and just as Carol continues, "Well, you didn't," in this shot facing Doug, the boom mic dips into view at the top of the screen.
