Visible crew/equipment: At the hotel's complimentary breakfast buffet, Paulie loads up a plate filled with Danish and Yoplait for the drive down to Florida. It then cuts to the Wycoff Therapeutic Center, and at the start of this first shot, we can see the boom operator (papers in the back pocket) with left arm up, at the right side of the screen. The reflection of the moving boom pole is also visible over the fireplace mantle.
Continuity mistake: When Tony shows up at Livia's house with a bouquet of flowers, in the exterior shot note the transparency of the screen door's glass, the front door silver-color knob with its back plate, as well as the deadbolt cylinder. As Tony enters the doorway in the next interior shot, we cannot see out through the screen door's glass, the door knob is gold-color, and both the knob's back plate and the deadbolt cylinder have vanished.
Denial, Anger, Acceptance - S1-E3
Visible crew/equipment: When Tony and the other guys visit Jackie at the hospital, there's an actor's tape mark (toe box type) on the floor, between the bed and the green chair.
Visible crew/equipment: After Dr. Melfi tells Tony, "I know seniors who are inspired and inspiring," when it cuts to Christopher and Brendan waiting outside the club, the red T-mark taped to the ground is visible at Adriana's feet.
Pax Soprana - S1-E6
Visible crew/equipment: When Mikey Palmice arrives at the poker game and begins to pummel Sammy, an actor's red tape mark is visible on the floor under the card table.
Continuity mistake: At the hospital, when Agent Harris presents his business card in case Tony changes his mind, Harris holds his card at its bottom so it's up over his hand, but in the next shot facing Tony, Harris holds the card at its top so it's below his hand, and it continues this way in the next shot as Carmela takes it from his hand.
Visible crew/equipment: After Paulie and Silvio hug AJ and Meadow at the hospital, when Meadow tells them Tony is with Carmela, we can see the boom pole and mic's moving reflection on the glass, at the top of the screen. Then, as AJ tells Meadow that he doesn't believe it was a carjacking, the mic's reflection dips into view again on the glass.
Visible crew/equipment: While Corrado and Mikey are at Mariolina's wake, just as Corrado says, "Where he spills his guts on some shrink's carpet," we can see the yellow tape mark on the carpet where Corrado had been standing.
Continuity mistake: When Christopher is trying to get rid of the body into the bin, his arms are constantly changing between crossed and hanging at his side. (00:30:30)

Trivia: When Paulie sits on a bar table in Naples and tries to greet some men by saying "Commendatori," one of the men is played by showrunner and series creator David Chase. (00:32:43)
Continuity mistake: When Tony goes to his cousin Tony Blundetto's house and talks about the incident involving Valentina, there's a circular object (like a sort of backpack) on the left side of the screen and on the couch, changing position in two different shots. (00:11:15)

Continuity mistake: When Tony watches Annalisa getting a pedicure from his hotel room's balcony, a glass table next to her shows a phone and a scissors case on it, changing position in two different shots. (00:29:35)
Revealing mistake: When Tony and Bobby are fighting at the cabin, Tony pushes Bobby against the glass door, which has a pre-break section visible right before Bobby crashes into it. (00:27:27)

Visible crew/equipment: When Sal chases Christopher after trailing him the night before, he knocks out a cyclist and crashes against a car. When he steps out of the car, the shadow of the camera and part of its crane approaching him is on his leg and on his arm. (00:36:20)

Continuity mistake: At Uncle Pat's farm, when Tony sits down to breakfast and the conversation turns to terrorism, when Pat says, "Not my problem much longer," we see the newspaper in Tony B's hands in the wide shot, but it cuts to a close-up and the newspaper is turned the opposite way, with no images and different body text.
