Doctor Who

Spearhead From Space - S7-E1

Other mistake: I know that regeneration causes the Doctor to change his appearance, but I didn't know it can produce a tattoo visible on the Doctor's right arm in episode 2.

The Creature from the Pit - S17-E3

Plot hole: In part 2 it's brought up in conversation several times that all metal is rare on that planet. Handy exposition for the viewers, but it's in fact a really odd thing for the characters to say. The inhabitants aren't giving a tour of their planet (certainly not deliberately at any rate), so why would they state facts that have been well known to them all of their lives? It's a bit like meeting strangers and informing them that two thirds of the Earth are covered by oceans.

The War Games - S6-E7

Continuity mistake: After the second doctor's regeneration, the TARDIS lands in a field with trees around it. When the third doctor comes out of the TARDIS, the background changes completely.

New this month Suggested correction: Correct me if I'm wrong here, but doesn't the Third Doctor not actually appear at the end of "The War Games"? As I understand it, the serial ends with Two being punished by the Time Lords by being made to regenerate, but the results of the regeneration aren't seen until Three arrives on Earth at the beginning of "Spearhead from Space."

The Three Doctors - S10-E1

Revealing mistake: Whenever the TARDIS's scanner picture of the Doctor's laboratory is shown, the TARDIS itself is always in the picture.

The Three Doctors - S10-E1

Factual error: It's stated that Omega caused the black hole when he created a supernova. Supernovas don't cause black holes, they're created by huge stars imploding.

New this month Suggested correction: Basic lifecycle of extremely massive stars: they're born, they live for several million years, they die in a large supernova. There are two potential outcomes of a supernova: the creation of a pulsar or neutron star, or the creation of a black hole, depending on how massive the original star was. So yes, black holes are indeed created in supernovas.

The War Games - S6-E7

Revealing mistake: When the resistance are storming the landing bay, one of them punches at least a foot to the right of a guard's face, but he still collapses.

The Web Planet - S2-E5

Visible crew/equipment: In an early TARDIS scene, at one point the cable powering the console is seen running from the base and under the wall.
