Shrink Wrap - S1-E8
Continuity mistake: When Doakes asks Deb "what took her so long" arriving at the first crime scene, Masuka is stood next to him. However, over the course of the conversation Masuka is seen in the background talking to another group inside the house, and in the next shot he is standing back next to Doakes. At the end of the conversation he walks over to the group inside the house. (00:05:30 - 00:06:00)
Revealing mistake: In the scene where Rudy's body is found by the police, you can see Rudy breathing and his pulse in his neck. (00:51:05)
Continuity mistake: In Season 3, Episode 2 during a conversation with Miguel's wife Sylvia, (Syl), and Rita in a kitchen, Syl says that they don't have any children after trying. In this episode, Syl is trying to help Dexter sell his condo and says that she downsized and focused more on "the kids."
Suggested correction: She could be easily saying that for the kids, as in those that they are trying to make happen, as a potential space for them if they get lucky and become pregnant.
Revealing mistake: When Dexter is retrieving a blood sample after cutting Benny Gomez's cheek, there is no blood in the dropper and he puts no blood on the actual slide. (00:49:50)

Factual error: In the police report Dexter examines, Emmett Meridian's name is followed by PhD, and yet he is referred to as a psychiatrist and prescribes medication. A psychiatrist would have an MD after his name, and a psychologist (PhD) cannot legally prescribe meds.
Suggested correction: It is possible for someone to be both a medical practitioner and have a PhD.
Someone with both a Doctor of Medicine and a Doctor of Philosophy would have both MD and PhD after their name. Not having an MD after your name means you can't prescribe medicine.
Yes, but in that case, both MD and PhD are listed after their name. Why would the report eliminate the MD, which is the more prestigious of the two degrees and certainly would have been noted during the investigation?
Revealing mistake: When Dexter enters his apartment and finds Zach dead in the chair, you can see Zach's chest moving, showing he's breathing. (00:49:45)
Other mistake: When Dexter is running Matt Chambers' fingerprint though his computer, he brings up four newspaper articles where the names Matt Brewster and Matt Rasmussen are used. In the final article, the headline reads "Matt Rasmussen of South Boston arrested", but the body of the article uses the name Matt Brewster exclusively. Other than the first few words, the text of the final article is identical to that of the previous one. (00:33:50)

Continuity mistake: The Ice-Truck Killer is responding to Dexter's Craigslist submission. The reply-to e-mail address is frozenbarbie@hotmail.web, but when in the new window that popped up when the ITK hit "Reply" the address is frozenbarbie@hotmail.com.
Shrink Wrap - S1-E8
Factual error: Dr. Meridian's victims are suicidal following withdrawal of an antidepressant, begging for more, and Dexter refers to the doctor getting his patients addicted to antidepressants. This reflects a disappointing and fundamental misunderstanding of the science of antidepressants. Even those that have "discontinuation syndromes" do not produce mood swings this wild. It is possible (though unlikely) that a random individual would have this reaction to discontinuation of an antidepressant, but Dr. Meridian would need to be in control of his victims' descent, to be sure they would become suicidal so that he could suggest the use of a gun and be sure their suicides would be successful. There are no mainstream antidepressants that would produce this effect upon withdrawal.
Other mistake: While Dexter's searching for info on Travis Marshall on his laptop, he opens the DMV Records window with an image of Marshall's driver license, and in the closeup at the bottom of the license it reads "Operation of a motor vehicle constitutes consent to any sobiety test required by law," with the word "sobriety" missing the letter "r." Additionally, under the image of the license it reads "Issued: 10-12-2006", but on the actual license image it reads, "DOB: 11-24-1977, Issued: 09-23-1990, Expires: 09-23-2012." So that means Marshall was not even 13 yrs old when he got his license, and the license won't expire for 22 years. Also, when Brother Sam knocks on the door Dexter clicks back onto the MOA website, and if you care to pause the screen note the word "Exibit" is missing the letter "h." (00:18:50)
Dirty Harry - S4-E5
Visible crew/equipment: After the ambulance taking Deb to the hospital drives away, a crew member ducks down in front of a parked car in the background. (00:04:36)
Audio problem: When Deb tells Angel to "look up" warning him of the security camera, Angel moves his lips as though he's talking but there is no audible dialogue. A few seconds later, his lips stop moving but then his audible lines are spoken. Then, again, there is no dialogue, but his lips move again. (00:07:15)
Factual error: When Dexter is reading the list of drugs found in the autopsy of the murder victim, one listed is "Cialopram." This is a misspelling of the antidepressant Citalopram; misspellings do not occur on drugs lists in toxicology reports, as they are computer-generated. (00:37:10)
Easy as Pie - S3-E7
Continuity mistake: When Dexter and Rita are having dinner at Miguel's house, his wife drops a glass. She sets 2 other glasses on the island in the kitchen. After a cut to another shot the 2 glasses are now on the counter by the sink, before she bends down and picks up the glass she dropped. (00:39:50)
There's Something About Harry - S2-E10
Continuity mistake: When Angel is drinking with Lila after finishing her apartment, the wine glass changes position throughout the scene.
Remember the Monsters? - S8-E12
Plot hole: In order to elude Elway at the airport, Dexter fills a backpack with random items and leaves it unattended under a seat, and then tells a desk clerk that he saw Elway deliberately put the backpack there and walk away, which leads to security detaining Elway and the airport being evacuated. Airports have security cameras everywhere, so security would have reviewed the footage and seen Dexter was the one who placed the bag there. They then would have gone through all security footage since he entered the airport in order to deduce his identity and then located and arrested him.
Dirty Harry - S4-E5
Other mistake: When Lundy is shot and falls to the ground, there is no visible injury even though he is facing the camera. Later, Masuka says he was shot twice, once in the chest and once in the neck. The shooter was facing him. We should have seen the blood.
That Night, A Forest Grew - S2-E7
Continuity mistake: The door on his fridge keep changing. When she says "pardon my tits" it is hinged on the right, other episodes it is hinged on the left.
Suggested correction: Hinges on fridge doors can be swapped around for ease of the user.
But virtually nobody casually re-hangs their fridge doors on a whim - they set them up once and leave them. This is clearly done for the sake of visual contrivance.
