Continuity mistake: They are hanging on the helicopter's rope, then they're still fighting on the roof and the helicopter's rope is being lowered. (00:37:50)
Other mistake: When the driver hits Sucre's car you can see a doll in the driver seat. And after the fight Gretchen gets in the car by herself and starts the car on the wrong side (the passenger seat), plus when she leaves the dead body disappears. (00:34:22)
Disconnect - S2-E12
Continuity mistake: Slattery holds out of the receipt for Brad to look at, she has her finger on the red tape. When the camera shows a close up, her thumb is off the red tape. (00:26:40)
Rendezvous - S2-E10
Other mistake: Sara sprints the short distance to Mahone's car. Halfway through her sprint the camera changes to Michael for 23 seconds, then back to Sarah, who has covered less than 10m in that time. (00:26:30)
First Down - S2-E4
Continuity mistake: When TBag is in the bathroom he has the blade against his finger. Camera changes and it isn't. (00:11:50)
Continuity mistake: We see Link driving with a blue short sleeved shirt on. When he arrives at his destination he's wearing a long sleeved grey undershirt. (00:14:20)
Other mistake: Near the start of the season Lincoln gets a bullet wound to his leg, However there is no exit wound, revealing the bullet is lodged in his leg. Yet he is able to run and move around with relative ease just hours after being shot.
Odd Man Out - S1-E12
Visible crew/equipment: When C-note's associates get back in their truck a stage light and equipment is reflected in the paintwork. (00:38:00)
