Continuity mistake: In the scene where Gibbs' phone breaks, and he tells McGee to "reboot" it, Tony is in the back, getting a phone out of the cabinet. He starts to give the phone to McGee. The next shot, the cabinet opens again, and Tony pulls out the phone again and gives it to McGee. (00:04:55)

See No Evil - S2-E1

Continuity mistake: When Gibbs leads Tony and Kate to MTAC, Tony extends his arm in a "ladies first" gesture to let Kate go ahead of him, then drops his arm to his side. In the next shot, his arm is extended again and he drops it a second time. (00:16:30)

Visible crew/equipment: While Sandy is helping NCIS by listening to the feed, Gibbs asks her if she can "remember anything else about the train sounds," and in the next shot we can see the reflection of the boom pole/mic (being held overhead) on the steel surface behind Abby. (00:34:05)

Visible crew/equipment: While in the morgue, Ducky asks Palmer if his recorder is on, and in the next shot a crew member wearing an orange shirt is visible on the right side of the screen. Continuity mistake: Caitlin opens the bathroom door of the apartment belonging to Burnett as demonstrated by the creak of the hinges. Camera cuts to inside the bathroom and she opens the door again. (00:09:15)

Visible crew/equipment: While Gibbs is interrogating Cluxton they are alone in the room, and when he shows her Harlan Wilson's death certificate, in the overhead shot as she begins to snap, the leg of a crew member is visible on the right side of the screen. (00:42:05)

Lt. Jane Doe - S2-E4

Plot hole: It really doesn't make sense that they found the real killer from Jane Doe #1 assuming he also killed Jane Doe #2. They only found him because they assumed he was in prison for the last 10 years. One could explain that he must have gone to prison shortly after the first murder because he said he'd be back but wasn't, but that he was actually recently released is just pure coincidence. And without finding him they would never have suspected Officer Cluxton.
