Continuity mistake: In the scene when Tony and Gibbs are riding together and Tony is eating a sandwich, it starts about half gone, the angle changes and there is only a bite gone, the angle changes again and it is about half gone again.
Visible crew/equipment: While Gibbs is with Abby in the lab when she mentions her dissection of the laptop, in the overhead shot as Abby says, "since it came in here, Gibbs," the moving wheel of the camera dolly is visible at the bottom, right corner of the screen. (00:09:35)
Continuity mistake: When we see McGee in the interrogation room for the first time, he has his hands clasped in front of him. When the shot changes, his hands are lowered. (00:18:30)
Continuity mistake: McGee is asked if he's looking to move to the private sector, and his hand is not by his chin. When the camera changes, his hand is now on his chin. (00:19:40)
Continuity mistake: When Gibbs is doing the interview near the end, he puts a photo on the desk. When the shot changes, the gap between the photo and the edge of the table is significantly reduced. (00:33:40)

Continuity mistake: At the start, when Diane is getting dressed, she puts her dressing gown on and does it up with her right-hand side over her left-hand side of the gown. When the shot changes, her dressing gown is done up left over right. (00:06:20)
Continuity mistake: When Eddie is shown the money, he passes it back to Fornell. This then moves back in front of Eddie in subsequent shots. (00:28:20)

Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
