Continuity mistake: When Lynette is chasing her neighbor out of her boys' bedroom and has a pillow in each hand, one is spotted and one is light blue. She raises the light blue pillow to throw at the neighbor. When the camera switches to the bedroom door, its the spotted pillow that hits the door behind the fleeing neighbor.

Continuity mistake: When Gabrielle fires Yao Lin, you can see that Yao Lin isn't wearing long sleeves, but when she passes Gabrielle, you can see that she's wearing a long sleeved white cardigan in the reflection of the mirror behind Gabrielle.

Continuity mistake: Episode 15- When the housewives are sitting on the porch discussing Mike Delfino, Teri Hatcher's green shirt keeps changing positions in the shot. It goes from down around shoulders to close to her neck and back.
Continuity mistake: When Bree is preparing a care package to ship to Andrew's camp, she sneaks some lemon bars into the package, and quickly closes the two side flaps of the cardboard box, leaving the end flaps out. In the next shot, she is immediately taping the box with all flaps down.
Continuity mistake: Parker is tearing the flowers apart until there are none. In the next shot, there are some flowers left, and when he jumps off the chair, the flowers have gone again.
Audio problem: When Lynette tells her sons to clean their teeth in the bathroom, the boys turn around and in the mirror you can see Lynette saying the same thing with no sound.
Live Alone and Like It (a.k.a. An Unexpected Song) - S1-E19
Continuity mistake: Lynette opens the door while she has sunglasses on. Then she is holding her glasses. It never showed her taking off her sunglasses. (00:13:20)
Audio problem: As Carlos is giving his speech at the barbecue, Julie sees Zach and says to Susan "Oh God". After cutting to a shot of Zach and his father it shows Julie and Susan again. If you look to the right of the shot you will see Carlos' face is just looking in front of him and not speaking, but you can still hear him giving his speech whilst he is in shot.
Continuity mistake: At the start of the episode, Zach picks up some gravel from beneath the lamppost and throws it at Julie's window. But when he leaves Susan the flowers, you can see that the gravel has disappeared.

Continuity mistake: When Gabrielle throws some salsa in John's face, it makes a mess, especially in the eye area. However in the next shot his face appears completely clean.

Revealing mistake: When Morty gets down on his knee to propose to Sophie, the angle of the camera shows the top of the set, which makes it seem as if there is no ceiling to the room.

Continuity mistake: When Bree is out for dinner with George, the scene starts with a close up of her eating with her fork in her right hand. It then cuts to a wide shot of her suddenly holding her fork in her left hand, and then she switches it to her right hand.
Plot hole: Rex is in the hospital for having a minor heart attack and is awaiting surgery to receive a pacemaker. 1) He isn't connected to an ECG or any other heart monitor so no one is aware of his heart attack. 2) They ran blood tests and checked his potassium levels. They would have also checked the concentration of his medication in his blood. (This should have been done earlier in the season too when they first discovered his medications were not working.) Zero concentration of medication in his blood could throw suspicions on his wife just as easily intentionally giving him potassium.
Factual error: This is for one episode 1-23. In the beginning, there is a shot of a calendar for March 1990. It shows March 1st being a Wednesday. March 1, 1990 was a Thursday.
Continuity mistake: In the episode where Gabby kisses Tom Scavo and, in return, Lynette kisses Carlos, Gabby is making a smoothie. When she's drinking it (as Lynette and Carlos are making out), it keeps switching from her left hand to her right hand, depending on which angle the camera is facing her.
Continuity mistake: When Bree is speaking to Lynette on the phone, Lynette asks Bree if she could borrow her scarf to wear at the job interview. But when we see Lynette at the interview, she has no scarf.
Continuity mistake: When Bree and Phyllis are talking about the funeral, the door behind the Reverend changes from open to closed between shots.
Continuity mistake: When Lynette throws the dirty diaper, it hits the glass wall and falls next to the trash can, but when the camera turns to Nina, in the background, the diaper is no longer there.
Answer: Kayla was sent away in episode 16 (The Gun Song) in season 4, after falsely accusing Lynette of child abuse. She now lives with her grandparents - probably Nora's parents.
I'm curious why we never see her again, why she never talked about, and why Tom never mentions visiting her.
