Continuity mistake: Right after Cordelia has her vision, Wesley goes to the whiteboard. In the "Case" column he writes "N.D.U.O." underneath what looks like "CARYNSS" crossed out. But once they find out that it is a Prio Motu demon and Angel writes it on the whiteboard, "N.D.U.O." disappears even though Angel did not rub it off.
Continuity mistake: After Wesley, Cordelia and Gunn sing "We Are The Champions" and Cordelia mentions tequila being evil, we see Wesley put aside some shot glasses. These glasses change position three times throughout the rest of the scene without anybody touching them.
Continuity mistake: When Angel writes "Prio Motu" on the whiteboard and says that they need to find it and narrow it down to somewhere, Wesley says "I may be able to help us with that." In the immediate closeup of Angel, his stance has suddenly changed. In the previous shot he was standing with his arms straight down and his hands close together, but in the closeup they appear to be as though he has his hands on his hips.
Continuity mistake: When Angel and Buffy are sitting down talking about their kiss, Buffy picks up a coffee mug and holds it with both her hands. But in a wide shot shortly afterwards, she is no longer holding the mug and it has moved further down the table.
Factual error: Connor has been raised since he was a few months old by Holtz, a man with a British accent, in a dimension where there are no other humans. There is no possible way he could have an American accent, yet from the moment of his return he speaks with one.
Continuity mistake: Right before Holtz and his men arrive at the hotel, Angel arranges three of Connor's toys in his cot. One of them is a teddy bear which he places in the middle of the cot. But once Holtz and his men arrive, the teddy bear is suddenly at the far end of the cot resting on the pillow.
Continuity mistake: In season three episode "Anne" of BTVS, The character "Anne" who later comes to season two of Angel running a youth shelter had a rather large red tattoo of half a heart on her arm. However, no trace of it is to be found when she is on Angel. Tattoo removal is a rather pricey procedure, and she seems to be tight on cash until she gets the two million. Even if W&H would have paid for the procedure, there would be some scarring or residual pigments for such a large tattoo. (It's definitely the same character - Anne says that she used to think vampires were cool, until she met one. That's a reference to her first appearance (as "Chanterelle"), in which she belonged to a vampire-loving cult.)
Continuity mistake: When Gunn is talking to Wesley near the start of the episode, he mentions that he's tracked Lindsay down, and that he's been living under the name "Doyle", and holds out the lease of his apartment. There's a close-up of the lease a few seconds later, in which the name "Lindsay McDonald" can be seen.
Factual error: According to the show, Angel learned quite a few languages when he was evil. However, when he tries to start a conversation with a red-faced demon, he makes terrible mistakes. He said "Hablo Espanol?" which literally means "Do I speak Spanish?" and he said something like "Are you from Byelorussia?" in a tongue that hardly resembled Russian, although according to the captions, it was said in Russian.

Revealing mistake: In "To Shanshu in LA" in season one, as Angel is pulling out of parking lot to follow the W&H Trucks, you can see his reflection in his rear view mirror (vampires have no reflections). Also odd that throughout the first season the rear view mirror is removed in some episodes.
Continuity mistake: In Season One Finale, "To Shanshu in LA" when Wes is lecturing Cordy on wants and desires, the number of bites taken from the doughnut changes, in one instance it breaks in half, most visibly when Wes takes a bite from Cordy's doughnut on the angle change.
Continuity mistake: In the scene where Angel has just returned and given Cordelia her necklace, watch Cordelia's hands; they keep changing position in each shot. In one shot she is trying to put the necklace on, in the next shot she appears to have given up and brings her hands down and bundles the necklace in her hand. Then when the shot changes to behind her, she is trying to fasten the necklace behind her neck again. This happens a few times during this scene.
Factual error: Wesley has his throat cut, deeply enough to at least nick the artery (based on the amount of blood) and the windpipe (based on his difficulties breathing and talking later on), yet he somehow survives being left alone in the park for over 24 hours. In reality, he would have been dead long before then.
Continuity mistake: In the season one finale "To Shanshu in LA," near the end Angel is fighting Vocah in the mausoleum and Angel rips off Vocah's mask, revealing the maggoty face underneath. In subsequent shots of Angel from behind Vocah, you can still see the edges of the mask on his face.
Continuity mistake: When Faith is sitting at the window and looks at the piece of broken glass she used to torture Wesley, it has a different shape than the one she first takes to torture Wesley. When she drops it out the window it has its original shape again.
Revealing mistake: In the scene where Angel fights the demon for the first of his trials, upon cutting the demon in half for the second time he chains the two halves to the walls. When he drags the upper half of the demon to the wall, the green legs of the actor are momentarily visible, when they should have been removed by CGI.
Revealing mistake: When Angel is seen gulping the blood from the cup (right before Lorne tells Angel that Connor needs some papa-love) you can see this is a special-effects cup (it's like one of those freezer mugs, where there's a layer of liquid on the outside). If Angel were really drinking out of the cup the way it's shown, he would have blood all over his lips and it would be running down his chin.

Factual error: When the mysterious person hands Cinco the business card during the 50's flashback (in the restaurant where he is sitting waiting for business), it says "Wolfram & Hart, Attorney's at Law." Clearly, there should be no apostrophe in "Attorneys" in this context - not a mistake a professional law firm would make.

Continuity mistake: In the season 5 episode 'Destiny' we see that the first time Angelus meets William, he has already adopted a more cockney accent, but in an earlier episode of BtVS (Fool For Love) Angelus asks Spike "Since when do you talk like that", suggesting he changed his accent after they met.
Character mistake: When the bidding for Cordelia in the auction reaches $20,000, somebody bids for $30,000 after the auctioneer has said "going once, going twice" and they end up winning. The auctioneer should have in fact restarted saying "going once, going twice" after the $30,000 bid was made.
Plot hole: After learning about the prophecy, the gang realise that Spike's headed off already, getting a head start. Angel grabs another car and heads after him, but why use a car? We find out in the first or second episode this series that he's got access to a helicopter, which he uses to get a long way ahead of some bad guys, and which would have saved a lot of hassle this time. Can't imagine Wolfram and Hart's pilots only work in the daytime.

Revealing mistake: When the gang attack the vampires in the bowling alley, watch as Jasmine and Fred walk over the bowling lanes in the background as the others fight the vampires. Jasmine slips on the floor but manages to maintain her balance. You'd think a deity with superhuman strength, telepathy, shape-shifting and psychic capabilities would be able to walk on a smooth floor without slipping and also know that it was smooth to begin with. Although she may lack friction, the shot should have been re-filmed as it is a blatant mistake.
Other mistake: Angel implies that Wesley told him the device will kill Illyria. But this Angel is from earlier in the timeline, as shown when he tries to tell Wes and Spike things they have just told their Angel. So Wes did not tell this Angel that the Mutari generator will kill Illyria. Angel should know nothing about it.
Character mistake: Angel comments that Vanessa was acquitted because of a hung jury. A hung jury means the jury couldn't come to a unanimous decision; the case would require a new trial, while an acquittal means the jury found her not guilty, and she couldn't be tried again for the crime.

Continuity mistake: When the gang rescues Angel and returns him to his normal form towards the end of the episode, Cordelia shocks him unconscious with a taser and he falls to the floor, lying on his front. But when Fred whacks him on the head with her baseball bat, he is suddenly lying on his back.
Plot hole: When Angel is being pursued by the Lilliad demons, he is holding his son Connor and is talking to him in the car. Later on it is revealed that he is in fact holding a teddy bear with a bomb attached to it. If it wasn't his son, why was he talking to it? It couldn't be to fool the Lilliad demons because they wouldn't be able to hear him, as they were in separate vehicles.
Factual error: Spike suggests that Illyria and Drogyn play Crash Bandicoot. They appear to be playing on an original Xbox console. The episode was broadcast on May 12th 2004, meaning that the only Crash Bandicoot game they could possibly be playing would be The Wrath of Cortex which came out on April 5th 2002 in North America because every other Crash Bandicoot game that came out before that was only on the Sony PlayStation (with the exception of Crash Nitro Cart, but they're not playing that one). Both Illyria and Drogyn appear to be playing, however, The Wrath of Cortex was a single-player only game. They're not playing Crash Twinsanity (available for the Xbox) either as that didn't come out in North America until October 1st 2004 and it too was a single-player only game.
Audio problem: During Lilah's failed attempt to rescue Angelus, when Gunn shoots at Angelus with the tranquilizer gun, Angelus passes out then hits the ground. When he is about to pass out, we hear the sound effect of him hitting the ground before he even begins to fall over, and instantly in the next camera angle he hits the ground.
Continuity mistake: When Wesley tells Faith that Angelus is back, she says, "Step away from the glass" and hangs up the phone. When Wesley jumps from his chair (to allow Faith to smash through the glass and escape) you can see Faith behind the glass with her hands up to her face and standing, but in the close up of Faith her hands are at her sides and she is only just getting out of her chair.
Continuity mistake: When Fred is running from the oncoming white car, she runs toward a blue car getting fixed on the side of the road, but jumps out of the way as the white car hits the blue car. Fred hits the ground and rolls over to the fence, but in a following shot it shows Fred only just hitting the ground and she is on the grass, not next to the fence.
Continuity mistake: When Jasmine picks up the car and throws it at Angel, it cuts to a shot of Angel moving out of the way then shows the car falling from the bridge. First mistake, Jasmine disappears from the shot when the car is falling, and second, the car falls directly straight and at a higher point than Jasmine actually threw it.
Other mistake: The Chinese writing shown tattooed all over Wo-Pang's body is nonsense (a small number of random characters repeatedly jumbled together), although Wesley reads it as if it is language. (Although one might argue that the writing is meant to be some sort of code, there is no evidence in the show itself to suggest that, and Wesley refers to "translating" rather than "decoding" it; for these reasons it is more likely to be simply a mistake rather than an intentional decision.)
Revealing mistake: At the vampires' nest, when Knox is talking about Gunn's team, the vampire wearing the green sweatshirt and leather vest doesn't have yellow eyes like the other vampires do. And the female vampire doesn't either later, when the two groups face off after Gunn's sister is sired.
Character mistake: While Gunn and Lorne are interrogating Framkin he sings a couple bars of a song "Courage and Pluck". However, though he has the ability to read people when they sing, and in fact in the very next episode knows that Fred is about to fall from just the words "you make me happy", Lorne doesn't read him.
Add timeLord Mordo
Continuity mistake: When the demon Angel and Groo are chasing crashes through a wall, the resulting hole lets in enough sunlight to force Angel into the shadows. A few shots later, Wesley notices Angel standing in the entrance to the hole. Only a few moments have passed, so Angel shouldn't be able to stand where he is.
