Corrected entry: General Grievous, whose two arms can split into four, has a total of twelve fingers (six on each hand when his arms aren't split). However in some shots (such as when he orders his ship's bridge crew to fire the emergency booster engines in an early scene) we can see he has five on each hand when his arms aren't split. In other shots, he has the correct number of fingers.

Correction: In the example that you mentioned, while General Grievous' right hand is mostly out of view, his left hand is seen, with six fingers on it. One of them was probably mistaken for another part of his hand, or a ship in the background.

Doctor Who (2005)

Correction: It has a round clockface both times. The only reason it appears to be different is due to illumination: in "Rise of the Cybermen", Big Ben is seen in daytime, with sunlight shining on it, making the clockface proper difficult to distinguish from the frame around it, which is ultimately square-shaped. At the end of "The Age of Steel", it's nighttime and the clockface is illuminated, making it easy to distinguish from the frame.

The Claws of Axos - S8-E3

Corrected entry: The Nuton power complex contains a cyclotron which, Winser explains, can accelerate atomic particles to the speed of light and beyond. There's even an instrument which measures the effect, with a scale marked in "x light". Unfortunately, relativity states that you can't accelerate particles to the speed of light (let alone beyond it), because at that point the particles would have infinite mass.

Correction: There is a theory that a cylinder of sufficient density would cause light to loop around it and accelorate. The particles wouldn't be breaking the local speed of light but would be breaking the universal one. The cylindrical nature of the chamber may generate this effect. This is science fiction so you have to assume that scientists found a way around the problem.

Corrected entry: When Ace is yelling at Mike for being a stinking dirty scumbag (the second time), she's supposed to be very angry. However, it's plainly clear that she's doing her best not to laugh. Near the end of the scene, she has a grin breaking through, and you can see her actually smirking as she runs off screen.

Correction: Ace was emotionally upset because of Mike, so was probably trying her best not to cry, which can be mistaken as laughter.

The Twin Dilemma - S21-E7

Corrected entry: As the Doctor and Peri prepare to go out onto the asteroid surface for the second time, the Doctor again heads for the door without operating the control. Thankfully, Peri says something else and the Doctor turns back for a few more lines, before finally going back to the console and hitting the real door control.

Corrected entry: When Luke, Ben, and the droids first enter the cantina and Wuher the bartender yells, "Hey. We don't serve their kind here." his voice does not match his lips. (His voice was dubbed by an American due to his thick cockney accent.).

Correction: I don't see how somebodys voice can match their lips, this is not a mistakes as everything is in perfect sync.

Correction: Who said Jack was suppose to be naked? It seems to me his tormentors took off his shirt and pants to expose some skin so they can inflict damage to them. Nowhere is there a code of ethics that says a torturer has to strip his victim totally naked.

Corrected entry: As Elizabeth is about to cuff Jack, a crew member is visible at the back of the boat. This is not an actor because all the characters had already abandoned ship.

Corrected entry: When the Pearl is chasing after the Intercepter to retrieve the gold medallion, the Interceptor drops its anchor into the water. As it catches and the ship suddenly dips forward the back of the ship is shown. The rear of the ship emerges from the water and you can see the propeller on the back of the ship spinning out of the water and splashing.

Correction: Vehicles with tires/treads can be seen in at least one of the Star Wars movies. Just because we don't see them onscreen is no reason to believe that wheeled vehicles don't exsist on Tatooine or Naboo.
