Continuity mistake: The godfather's thugs storm Nick's loft and riddle him with bullets. We actually see the bullets coming out of his back, leaving holes in the back of the vest he wears. But when the "vamped out" Nick turns around to face the camera, there are no bullet holes in the front of his vest. (00:23:20)

Continuity mistake: While the doctor is playing the tape for Reese, Nick and Tracy, the Toronto Leafs hockey team coffee cup on Reese's desk is sitting with the leaf logo turned away from the camera and the hockey player art showing. A few shots later, though Reese isn't drinking any coffee and hasn't touched it, the cup turns itself so the leaf logo is now visible. A little later, it's turned back again. (00:22:30)

Continuity mistake: Nick and Schanke are interrogating a murder suspect and disagreeing about whether or not she did the deed. They leave the interview room together, and Nick's hair completely changes styles as he walks out of the room. On the inside of the door, it's combed down over his forehead and rather "fluffy." The minute he exits on the other side of the door, it's brushed back and slicked down. (00:25:55)

Shore Leave - S1-E16
Continuity mistake: Don Juan tears Yeoman Barrows' tunic at the right shoulder. Near the end, when she changes from the fairy tale dress back into the tunic, the torn right shoulder is suddenly mended. They're cut off from the ship by the power drain, so she couldn't have beamed up for a new one. She emerges from the bushes after changing, and now her tunic is torn at the left shoulder. (00:43:25)

Continuity mistake: Finnegan flips Kirk over his shoulder near the end of their fight. When Kirk lands, his uniform is still completely intact, but when we cut to a close-up, his shirt is suddenly shredded. (00:38:30)

Continuity mistake: Orac is in two places at once: he's sitting on the console, plainly visible in front of Dayna as she calls the landing party - but in the next shot of the crew over on the orbiter, Vila is holding Orac. (00:40:45)

Continuity mistake: In his scuffle with Servalan's trooper, Vila's teleport bracelet falls off and rolls away. We're even shown a close-up of it as it comes to rest near the dropped gun. Yet in the very next shot, as the fight continues, the bracelet is back on Vila's wrist. (00:37:20)

Continuity mistake: As Balok's 10 minutes are counting down, McCoy comes onto the bridge wearing a standard blue velour uniform. A few minutes later, when Spock says he can bring up Balok's image, McCoy is suddenly wearing his medical tunic with the different fabric and collar. Then when Bailey starts losing it, McCoy is back in the regular velour costume again.

Continuity mistake: The planet Terminal changes shape and color completely from its appearance in the previous episode. Where it was blue-green and cigar shaped before, it is now gray, striped like a yarn ball, and round. (00:07:00)

Continuity mistake: Avon apparently found a hair salon somewhere on the supposedly deserted planet Terminal. Between the time he and his crew were marooned in the previous episode and the events in this one, he's suddenly acquired a spiffy new blow-dry haircut. (00:03:10)

Continuity mistake: In the final scenes on the planet, Blake's teleport bracelet appears and disappears repeatedly between shots. It reappears the final time just before he snatches it off and tosses it away to avoid being teleported up. (00:44:35)

Continuity mistake: When the crew teleports up at the end, the shirtless Vila arrives in a completely different position relative to the others. On the surface, he's standing in a ring with the rest of them. When they materialize, he's hunched over and is now in the middle of their circle with the others around him. (00:42:25)

Continuity mistake: At the end of "The Way Back," Blake, strapped in his prison ship seat, looks over his shoulder at the window behind him as the ship leaves Earth. This same scene is recapped at the beginning of "Spacefall," only now Blake is sitting one seat to the left of the window instead of directly in front of it. (00:49:00)

Mr. Monk and the Girl Who Cried Wolf - S3-E6
Continuity mistake: About midway through, the new nurse is sitting on Monk's kitchen counter eating a sandwich. She puts the sandwich down on a plate as she hops off, the shot switches to another angle to include Monk as she walks away, and the sandwich has suddenly jumped off the plate and is now lying on the counter a few feet away. Neither of them had time to pick it up or move it.

Continuity mistake: Vila makes an improbable costume change in this episode. He's wearing one outfit at the end of "Terminal," when Liberator is destroyed, and another outfit entirely here, where he's supposedly marooned on the planet with the remainder of the crew. Did he take time to smuggle a spare set of clothes off the ship, even though it was disintegrating at the time? (00:03:30)

Continuity mistake: In the flashback scene of the kids in the school hallway, the autographed photo of Patty Duke disappears and reappears twice on the inside of Little Monk's locker door as the camera angles change.
