The Night of the Vipers - S3-E18

Revealing mistake: The final gun battle in town has a gunman in a tower shooting at West. The gunman's perch is enclosed on all four sides by a bannister, but when West returns fire, shooting the gunman out of the tower, the bannister in front of the gunman has disappeared, allowing the stuntman a clear path to fall out of it.

Stupidity: The Engineer and Fireman of the Polar Express train crew should have known better than to not bend back the the metallic prongs of the cotter pin, which would have prevented the cotter pin from coming loose and causing all those problems in the driving of the train.

Suggested correction: As you yourself stated, the whole thing is likely a dream, where "normal" reality doesn't apply. In the "real world", the train would never be able to do any of the things that it does in the film.

Dream or not, it is still a stupidity of the train crew to not secure a pin that could work itself free of the controls of the locomotive.

The entry doesn't say anything about the entire trip to the North Pole being a dream.

Agreed. It was never stated that it was a dream considering that Hero Boy lost the sleigh bell only to find it Christmas morning with a note from Santa. And to add, Steamer said that cotter pin was sheared off which caused it to come loose.

Revealing mistake: When Snots the dog chases the squirrel into the Griswolds' dining room, he barely brushes a high-back dining chair that immediately falls on its side. If you look closely, a string attached to the the right side top of the chair can be seen tightening which then pulls the chair down.

Continuity mistake: When Clark, Eddie, and Catherine are admiring the Christmas lights of the Griswold house, Clark is very close to Eddie with his left arm draped over Eddie's shoulder, but the next quick cut to Clark when he says, "Eddie?", shows Clark further away and his left arm at his side, and not stretched out.
