Corrected entry: The Reaper (acting as a police officer) asks for the driver's licence, registration and proof of insurance. This episode takes place in Massachusetts where proof of insurance is on the registration. Police only ask for licence and registration.

Corrected entry: Near the beginning we see Ray drinking a cup of Dunkin' Donuts coffee. There are no Dunkin' Donuts coffee shops in the greater Los Angeles area.
Correction: Ray just reused the cup that he had from a prior purchase. There's no reason why he can't have been in another city on business, at some point in the past.
Correction: At that time there were no Dunkin Donuts in California.

Corrected entry: Cole picks up a picture of Phoebe. In the next episode Phoebe is wearing the same outfit and hairstyle.
Correction: The outfit that Phoebe is wearing in the picture that Cole picks up in "The Seven Year Witch" from season 7 is not the same outfit or hair in the next episode which is "Scry Hard" I just got done watching both episodes on TNT this morning and went back through all of my DVDS of the show and that outfit and hair of Phoebe's is from the episode "A Knight to Remember" the one where Paige is the evil Enchantress from season 4 and the 5th episode.

Corrected entry: In "A Hole in the World," while Fred is laying in the infirmary she calls Angel "a big, handsome man who saves me from the monsters". Those are the exact same words that she called Angel when he saved her in Pylea in season 2.

Corrected entry: In the first episode when Phoebe is in her room, you can see that the tattoo on her shoulder has covering makeup on it, but not enough to completely cover it. It's not covered up for the rest of the series.
Correction: People cover tattoos with makeup all the time. It's possible she wanted to cover it but later decided she didn't care.

Corrected entry: Dr. Troy disrobes so that a dermatologist can examine him for moles. He said he often tans in the nude. Later on we see his bottom end and it is quite pale. Doesn't match the tan on the rest of his body.
Correction: "Often" doesn't mean anything close to "always" and "tans" doesn't always take into consideration basic outdoor activities in which one obtains a tan, especially in Miami. His body is so tan in comparison to his pale bottom that "often" may not have been enough to offset the difference. Also, depending on lighting, say in a doctor's office, shower or poorly lit bedroom, where we most often get to see said bottom, this difference may be exaggerated.

Corrected entry: In the episode "A Hole in the World" there's a connection between the Angel and Spike argument about who would win in a fight between cavemen vs. astronauts and Fred becoming Illyria. Like Illyria the cavemen are old and powerful while Fred is a scientist like astronauts. A little before she dies Fred says of course the caveman would win. She then dies and is taken over by Illyria.

Corrected entry: When the girls are on the ground after being shot off the roof by Madison Natalie's eyes are wide open as if she were dead. Actually she was unconscious so her eyes should be closed.Or, at the very least, blink which is involuntary.
Correction: People can sleep with their eyes open. People can go unconscious with eyes open.

Correction: As you said in the entry, the Reaper was just ACTING as a police officer. Its entirely possible this wasn't known, as he may not have ever been pulled over.
