Revealing mistake: When Gretchen gets run over, you can tell that a dummy was used as the car's wheels pass over her. The position of her head changes between shots and her body seems a little too still for a living person who's just been run over. She should have been tossed a few feet or maybe even gotten caught under the car's wheels.
Revealing mistake: When Yen jumps backwards onto the shelving unit during the con, you can hear a crew member say, 'Left' mid-jump. (01:26:45)
Revealing mistake: When the Lawyer plays the videoclip on the laptop, the stop button is pressed down all the time, instead of the play button. (00:14:40)
Revealing mistake: During the scene where the tractor trailer truck crashes through the ice truck in the field, you can tell that the ice truck has been lengthened to make this possible.
Revealing mistake: When Kakihara is smoking in the beginning, you can tell the cigarette is not really lit, and that the glow of it was added later using computer graphics. The easiest way to notice this is to notice how no smoke is coming off of the cigarette itself.
Revealing mistake: In the shootout scene on the bridge, when the helicopter is being shot by Steven S., you can see that each time a bullet strikes the helicopter, it splinters, revealing it is in fact made of wood.
Revealing mistake: At 1:20:25 - Just after Joe and Bobby have returned to the plane to pick up the gold, we see 2 ground crewmen walking across the front of camera. One of them stares directly at the camera for a half-second, before shaking his head away, so as not to look like he's staring at the camera. (Danged extras. .). (01:20:25)
Revealing mistake: When Jack breaks into the Albert Dock Road building where the hijackers have their hideout, he enters through a wire-mesh-reinforced roof window. From below you see that he hits it once, causing a huge shower of big glass pieces (as if there were no wire mesh), and then the whole, almost intact pane comes down as if the mesh hadn't been connected to the frame. Revealing mistake: In the scene where the prisoners are moving their blockade of dinner trays across the yard, you can see one of the prisoners run too far to the right.
Revealing mistake: When Paul is in the car trying to escape, he hits one of the villagers. The man is obviously yanked back by a harness before the car hits him.
Revealing mistake: In the last shot of Elwood machine gunning his Mercedes, he's run out of ammo - there's no belt left.

Suggested correction: Sometimes there are many minutes, or even an hour, in between scenes that would have passed in universe. More than enough time for a quick rainstorm to roll through off camera.
Revealing mistake: Just as Liz goes to talk to Mike (where she talks about slitting their wrists with Geoff's knife) Geoff is in the foreground of the shot, covered in a coat/blanket/something, and is supposed to be dead. Suggested correction: Cyrus was an occultist, it is presumed that this video was haunted. That can also explain why he answers what they key is for in the video after someone asks what it's for.
This theory does not hold up because Cyrus is alive. He's just been wearing a prosthetic neck piece with a popped blood pack in it to appear as a ghost the whole time, in case he's seen walking the house during the plan.
