Continuity mistake: When Trish is talking to Jess on the phone, she pours herself a glass of water and places the bottle of water down to her left. In a brief shot just before her nose starts to bleed, the bottle is suddenly to her right, and then back to her left. (00:45:25)
Continuity mistake: When Kimiko says "Will you two please stop abusing the English language!", Raimundo's eyes are blue.
Continuity mistake: Near the end of the show, the squadron has to land on a carrier deck. The shots from the sky go back and forth between a straight deck carrier and an angled carrier deck. Also, an angled carrier wasn't put into use until the early 50's by the British, Black Sheep takes place in WWII.
Continuity mistake: (Season 2 Episode 2) Near the end, as the worms have died and exploded trying to spawn, a shot shows Cutter, Claudia and Stephen ripping the spawning worms from their bodies. Stephen is shown with one in each hand and another hanging on the right of his chest. In the following shot he is seen ripping one off his right chin, which did not have any worm on it in the previous shot.
Continuity mistake: During the car chase when the camera is inside the Sweeney Granada, a green Fiat can be seen driving in the other direction. The shot immediately changes to outside the car and the Fiat has gone.
The Dark Phoenix (2): The Inner Circle - S3-E12
Continuity mistake: Rogue rips off Donald Pierce's cybernetic left arm, but when he is first shown running away, his right arm is the one that is missing. When he catches up with Sebastian Shaw in the secret passageway, his left arm is back to being the one missing. (00:18:30)
Continuity mistake: Rick pulls up in a car to the right side of a helicopter, gets out, and gets in the helicopter. When the helicopter is shown taking off, there is no car next to it. (00:38:20)
The Moulin Ruse Affair - S1-E17
Continuity mistake: From the outside, the U.N.C.L.E. building's wall is made of red brick. But, after a large hole is knocked through it, a shot from the inside shows no red brick at all. In fact, the entire wall is now made of white cinder-block. (00:02:30)
Continuity mistake: When Moreno is trying to get the border patrol cops to call for a helicopter, we see one has his arms around the other's neck, but in the next shot, his arms are around the other's waist.
Continuity mistake: Whenever Banner changes into Hulk his clothes rips, only his pants stays on. But when Hulk changes back to Banner, he suddenly has all his clothes on, no rips or tears at all.
Left Behind - S3-E10
Continuity mistake: In the previous episode Ra's al Ghul stabs Oliver through his chest, just below his left pec. When he pulls out the sword, a wound is visible. (However, seconds later, the wound is absent - different mistake). In this episode he is shown with a wound much lower (in his lower abdomen) and also later, with a bandage around his waist instead of his chest. (00:09:30 - 00:41:30)
Continuity mistake: In an earlier season Caine takes a young Peter to a place in the temple and explains that his mother is buried there. However in the series finale he shows adult Peter a picture that someone took months ago and Caine tells Peter that the woman is his mother and he's going to find her.
Continuity mistake: When Marion James has the conversation with Rawlins on the bridge, Rawlins by all miracles isn't blind at one eye anymore. He does still have the scar next to his eye. But the eye itself is okay. (00:44:55)
Continuity mistake: Throughout the series, the Rangers' morphing has been instantaneous. Anyone who has ever been in a photobooth will know that they take a good minute or so to get through all 4 photos (which is what Sophie picks up) - it has never taken Carlos, or any Ranger, that long to morph.
Eyewitness - S1-E12
Continuity mistake: When the immortal who's trying to kill Tessa snipes at her in Duncan's car, his shot shatters the passenger-side window. When Duncan tails Tessa and her police escort to the safehouse, the passenger-side window is intact. (00:30:05 - 00:35:50)
Continuity mistake: In the episode "The Education of Jaime Sommers", when Jaime and Tom are having coffee, the size and position of cappucino foam on Jaime's nose changes from shot to shot.
Continuity mistake: In episode 3, "How Billy Got His Groove Back", in the scene where Mick is meeting with Eduardo on the overpass Mick's sunglasses keep switching between being up on his nose to down on the tip of his nose to being off his face and back on again.
Too Many Christmas Trees - S4-E13
Continuity mistake: When Steed and Emma are driving to the estate, the car's windscreen keeps disappearing. It's there in full shots, gone in close-ups, and reappears again every time we cut back to a long shot. (00:10:30)

Continuity mistake: Jack's father the emperor is a young man when he defeats Aku. At the conclusion of the movie, he greets his pregnant young wife. At the start of "The Beginning" movie, Jack is a very young boy, both his parents are well past middle age-and Jack has no sisters or brothers to explain the gap.
The Return of Optimus Prime (1) - S3-E29
Continuity mistake: At the end of the episode 'Dark Awakening', Optimus is seen without an eye and an arm, he also has other damaged points, but at the beginning of this episode, when Jessica and Gregory get to Optimus, he is in perfect condition with no damage.
