Kanalu Hope Loa - S5-E3

Continuity mistake: When Kono shoots the bad guy who then falls off the balcony and lands on the hood of a truck, his head is toward the front of the truck. Later when they bring out the female leader of the gang the guy is lying across the hood.

A'ale Ma'a Wau - S4-E2

Continuity mistake: A Texas Ranger spreads peanut butter on a suspect in a sugar cane field. The suspect is tied up on the ground and mostly on lying his back and right side. He spreads the peanut butter on his left arm, head and chest. In a later scene, the suspect is taking a shower and you see lots of peanut butter on his back, both his arms and the back of his head.

Ko'olauloa - S1-E6

Continuity mistake: At the end of the episode, the surfers are in the bay to spread Ian's ashes. Ben lets go of Kono's hand and is handed the urn with the ashes by the man to his right. He removes the lid of the urn and hands only the lid back. Yet in the overhead shot immediately following, the urn is back on the board of the man to Ben's right, and there is nothing on Ben's board. He's holding Kono's hand again. In the next shot, he is tipping the ashes from the urn.

Continuity mistake: Steve is talking to a female suspect. In one shot, her hair is off her face and the light source is strong from the left. Next shot, her hair is almost covering her right eye and the light is more diffuse and not as strong.

Continuity mistake: Steve and Danny are talking to a murder suspect. She steps out of her office holding a folder in her hand. Shots alternate between her holding the folder and having her arms crossed.

Ohana - S1-E2

Continuity mistake: In scene where Kono makes origami crane, she sets it down on top of Evan's Ipod. Next shot the crane is sitting next to the Ipod. (00:26:30)

Ohana - S1-E2

Continuity mistake: In scene where Chin starts reviewing footage of white van attacking, in first shot a wheel from the NSA guy's van has rolled off and comes to rest immediately in front of attacking van's driver side front wheel. In next shot as driver exits, loose wheel is missing. (00:11:20)

Ka 'i'o - S10-E7

Factual error: The plane taking McGarrett from Panama to Mexico purports to be owned by a Mexican fishing company. But the registration number on the plane starts with an N and has 5 numbers and a letter. First of all, a Mexican aircraft would not have a registration number starting with an N. That is reserved for aircraft of US registry. All that aside, the registration number is invalid. US aircraft have at most five characters after the N. The registration - as I suspected - was shown to be in an invalid format when queried in the Federal Aviation Administration database. (00:11:18)

Ke iho mai nei ko luna - S9-E13

Other mistake: A Five-0 member makes an atmosphere check of the Neptune 1 air. It shows 0.7% oxygen, 80% helium, and 16% nitrogen. A previous check had shown these were the only gases present. So what happened to the other 2.3% of the air? Helium and Nitrogen should have been proportionately higher so as to add up to 100%. (00:29:45)

Factual error: In Steve's office behind his desk is a model of CVN 65 USS Enterprise. The island superstructure is back to front with the Bridge actually facing the rear. This glaring error is present in all episodes to date.

Ke Kinohi - S1-E13

Other mistake: An important key to the first season's plot is the "dancing men" code on the postcard. We see the postcard several times leading up to Ep 1:13. It shows 3 lines of code containing 4, 4, then 3 characters. In this episode, however, Steve brings out his workings on a yellow legal pad to show the others. There are more characters on his pad than are on the postcard, and some of the characters (for example those with both arms on one side of their bodies) do not even appear in his workings. He claims to have used frequency analysis to work out the code. If one uses frequency analysis on the actual code we see on the postcard, the result can never be Hiro Noshimuri. (00:23:35)

Ho'ohuli Na'au - S1-E22

Character mistake: Kono and a lab tech are stopping at several stations to sample their 87 Octane gas, yet at the third gas station he uses the middle pump that reads "Plus" which would be 89 octane - the wrong gas. (00:19:10)

Ko'olauloa - S1-E6

Continuity mistake: When McGarrett chases the bad guys' Jeep, his Camaro skids through several huge dust clouds. Immediately after each cloud, however, the car is perfectly clean again. This is visible in several of the very rapid shots throughout the chase. After the chase ends, a coating of dirt and mud (which, incidentally, differs from the earlier shots) appears on the Camaro.

Ka Ho' Oponopono - S2-E13

Continuity mistake: When the teenagers look up at Emily's window, it's half open. When they climb to it, it's completely open. When McGarrett arrives, the window is half open again. (00:05:00)

Question: Why did Tommy chase after Kelly with a loaded weapon in his hand towards the police after she ran from his car towards them? Common sense would have said that to do so meant the police would have no choice but to shoot to kill.
