Continuity mistake: When Meredith puts the Hello Kitty plaster on her forehead in her car, she puts it on upside-down. When she reaches the elevator it is now on the right way up (it is unlikely she would have removed it just to place it back on the right way up.) Continuity mistake: At the very start of the episode when it shows Burke laying on the floor after being shot, there is a very large pool of blood with his glasses right next to the blood. But in the next shot there is no pool of blood anymore but it returns in the next. Additionally, the pair of glasses change position. (00:00:05)
Continuity mistake: You see Karev in the elevator with O'Malley putting on OR scrubs, in the next scene Karev isn't wearing his but a few shots later he is. Karev is wearing gloves, yet we don't see him putting them on.
Continuity mistake: Meredith takes over injecting George for Alex, and right after she stands behind George, Alex leaves. Just when he opens the door, suddenly you can see Alex is behind George and Meredith is just coming in again. (00:19:55 - 00:20:50)

Continuity mistake: When Addison and Alex are talking to Rose about tying her fallopian tubes, her necklace jumps from under or over her shirt between different shots. (00:08:45)
Continuity mistake: When everyone is in the locker room nearing the end of the episode Alex starts to shout at George. As he does, George removes his white coat twice between shots. (00:37:20)

Continuity mistake: After Meredith puts her hand on the bomb, everyone stands up. It shows Christina and Burke standing up looking at her, But in the following wideshot Dr. Burke is suddenly crouched down again.
Continuity mistake: At the beginning, in the car on the way back from the hospital, in one shot Dr. Kepner is at Dr Karev's right and in the next she's at his left.
From A Whisper to a Scream - S3-E9
Continuity mistake: During the operation on the Asian girl from the market, a stitch breaks and Christina is covered with spurting blood, including all over her mask. When she turns to leave the OR, the all-over splatter on her mask has turned to dribble-like streaks instead.
Continuity mistake: Derek drives in to talk to Owen. He gets out of his Porsche Cayenne and closes the driver's door. In the next shot you see the door open in the background.
Continuity mistake: Arizona is wearing fitting black slacks and black heels and has two legs (not a prosthetic on the left). (00:37:45)

Continuity mistake: In the first overhead shot showing Izzy lying under the Christmas tree, you can see Meredith's hair to Izzy's left but then the the next shot is Meredith coming into the room and Izzy under the tree by herself. (00:41:05)

Continuity mistake: When Christina notices the bomb squad moving Meredith down the hallway with the gurney, she leaves the room and removes her scrub jacket and hands it to a nurse. When she is out in the hall and reaches them, she suddenly has the scrub jacket back on again. (00:18:50)

Continuity mistake: When Izzie starts kissing Alex in the closet, as he tells her to stop she rests her hand on the back of his head. In the next shot it is on his cheek. (00:09:45)
Continuity mistake: When Derek talks with Mr. Chue in the rain under their umbrellas, in one shot Mr. Chue takes a big puff on his cigar. But in the next shot his face is relaxed and it is clear he didn't inhale; and then he takes the same puff again instantly anyway. (00:19:20)
Continuity mistake: As Meredith tries to get in to see Christina, but Bailey walks over to stop her, Meredith is putting on a white surgical mask. In the frontal shot she ties the white straps of the mask into bows. But in the next shot from behind the straps hang loose and aren't in bows. (00:29:50)
Goodbye (2) - S6-E2
Continuity mistake: In episode 6-02, "Goodbye," in the scene where Izzy is getting chemo, the amount of popsicle that is bitten off on Meridith's pop changes from more to almost nothing from shot to shot.
Continuity mistake: Dr. Hunt brings a clear drink over to Christina. In the next shot, Christina takes a sip of the drink and it is dark. In the next shot, the drink is back on the table and it's clear again.
What Have I Done To Deserve This? - S2-E20
Continuity mistake: At the beginning when Meredith is laying on her bed, her head is dead centre on her pillow. In the next closeup her head is right on the edge, almost falling off of it. (00:01:05)
