The Lion Sleeps Tonight - S8-E18

Other mistake: At the beginning of the episode, when Meredith, Derek and Zola see the lion crossing the street in front of the car, the lion roars and it is clear that the lion is not missing any teeth. However, later in the episode, Richard removes a tooth that had gotten lodged in the male mauling victim's arm. The victim was the lion's owner and the attack happened in his house, so by the time the lion was out the tooth should already be missing.

All You Need Is Love - S8-E14

Other mistake: There are two mistakes when Grey is at the computer trying to figure out why her patient is having seizures. The camera pans out showing Bailey walking up - if you look at the computer it shows a login screen, but when Grey shows Bailey the screen it changes. Next one is when Grey blows up the pic of the taratoma - when they leave the picture on the computer it's actually a brain.

How to Save a Life - S11-E21

Other mistake: The exact same shot of Derek's radio tuner is used twice-once when he calls Amelia and again when he gets in the car after helping at the accident. Both times, the clock says 7:45PM, but Alana (the AP class taking, cello playing student) says that she's supposed to be at school, implying it's sometime between 8AM-ish and 3PM-ish. Also, the sun sets around 7:30PM in Seattle on March 26, but it is obviously day time for the duration of this scene so there's no way it could be 7:45PM in either of the shots in which the clock is shown. I'm Winning - S10-E19

Other mistake: When Derek has finished showing Callie and Arizona the picture of Cristina getting her nomination, he leaves to take the kids into what we are supposed to believe is daycare...however the sign on the door reveals it is a dispensary. (00:02:30)
