The Pilot - S10-E2

Other mistake: After the Doctor tells Bill that Heather's face isn't symmetrical, when he and Bill look into the "puddle" together the Doctor explains that instead of seeing her normal backwards reflection, she's seeing her face "the right way round." The problem occurs in the next shot of the Doctor and Bill seen in the puddle, because the "WOW!" patch on Bill's jacket is properly where it should be, but note everything else is actually backwards. Things such as Bill's color-block top and her denim jacket buttons are on the opposite way. Also note the Doctor's ring has switched hands. Presumably this shot was edited, and the "WOW!" patch was blurred on the left of Bill's jacket then digitally added to the right side, to give the appearance that it's "the right way round" as the Doctor said. Blink - S3-E10

Continuity mistake: When Lawrence is watching the Weeping Angel, just after seeing the Doctor's film/Easter egg, he turns away for a split second. By the time he turns back, you see the Weeping Angel's face only inches from Lawrence's face. But you then cut to a wide shot where you see the Angel's arms outstretched, so its face is about half a metre away from Lawrence's face. (00:34:45)

Continuity mistake: When Mr. Copper is talking about how space-lane fraud could get him ten years in jail, the cut on his forehead changes sides, as does the panel behind him, revealing the shot to be flipped. (00:36:25)

Revealing mistake: At the end of the episode there is a side shot of the Doctor leaving the TARDIS in which you can see inside, but the control room isn't visible, just the side of the police box, with light from outside shining through the window. (00:38:00)

Revealing mistake: When Dalek Sec picks up the remains of the radio and turns it in his hands, the "Hamilton" label on the front of the radio is written backwards, indicating that the shot has been flipped horizontally. (00:05:00)

Continuity mistake: At the beginning, when the Doctor takes Amy to planet One, he holds out his hand for Amy to take hold of when they're about to leave the TARDIS. When you see her from the back, Amy lifts her hand wearing her red fingerless gloves. But when it switches shot and faces Amy from the front, she takes the Doctor's hand wearing no gloves. When she steps out of the TARDIS, her gloves are on again. (00:04:54)

Revealing mistake: In the opening scene the Doctor and Donna are speaking over the phone. The purple phone Donna is holding shows the slot where a cable should be plugged in, but there is none. (00:00:25)

Suggested correction: This shouldn't be considered a mistake. While it's true in real life this phone has a slot for a cable, they're on an alien planet and this is unknown technology. In-universe there's no way of knowing what that slot is for, and there's no reason to think the phone needs to be plugged into anything.

Continuity mistake: As the camera zooms out when Rose is captured and Cassandra is about to "go" into Rose, the psychograft disappears, but in the next shot of Rose it appears again. (00:11:45)

Trivia: "Torchwood" is an anagram for Doctor Who. Russell T Davies came up with it during the filming of Series 1 to label the tapes in order to prevent theft and potential leakage, and decided to use the name in the show proper.
