Mr. Monk Makes A Friend - S5-E11
Continuity mistake: The position of Gail's body moves at the crime scene from when officers cover her head with the tarp to when the Captain uncovers her a few moments later. At first, her left hand is under her forehead and covered by her hair, but then the hand is away from her head and not covered by hair.
Mr. Monk and the Other Detective - S4-E1
Continuity mistake: When the reporter is interviewing Marty's mom at the crime scene, the reporter lifts the mic to her mouth, but in the next shot the mic is away from her mouth.
Mr. Monk and the Election - S3-E15
Character mistake: Captain Stottlemeyer uses gloves to pick up the letter found sticking out of the pipe. He doesn't actually put the glove on, but still uses it to avoid getting any of his prints, skin, etc on the paper. But then he uses his other, non-gloved hand to open up the note and hold it, and Monk also touches the letter. If he cared about not contaminating evidence by using the glove, he wouldn't touch it bare handed and if he didn't care about touching the note bare handed, then he wouldn't have used the gloves.
Mr. Monk Goes Back to School - S2-E1
Other mistake: Derek tells Beth to meet in their "usual spot" the next day at 8am. The next scene has Derek sitting at his desk and it looks like the kids are already in deep with their SAT tests. Beth falls at 8:25am (as per the clock in the testing room.) There is no way that Derek could have killed Beth, put her on the minute hand (later in episode on the Monk recap, it has 8:15am. If Derek was proctoring a SAT test, I don't know of any that start around 8:20 and immediately in depth.
Mr. Monk Goes Back to School - S2-E1
Factual error: In the cold opening, Derek said that his wife is very smart and got a 1680 on the SAT. This episode takes place in 2003, the writing portion that was added to make the max (2400, not 1600) was in 2005.
Character mistake: It seems throughout the whole series at crime scenes Monk, Randy, and Stottlemeyer handle evidence bare handed. Surely, they should be wearing gloves so as not to contaminate the evidence.
Mr. Monk Goes to Jail - S2-E16
Factual error: The murdered man is killed by poison while having his last meal just before they execute him that night. Death row inmates have their last meal one to two days before they're executed.
Mr. Monk and the Big Game - S5-E3
Factual error: All the players have their first names on the back of their jerseys. They should have their last names on the jerseys for better identification considering some of them would probably have the same first name.
Suggested correction: This isn't a mistake just because a school does it a different way than what you expect. Not to mention that some people also have the same last name, so using the last name would create the same "problem." All they would have to do is add a last name initial.
Mr. Monk and the Birds and the Bees - S6-E5
Other mistake: Monk wasn't afraid of heights, as he climbed on a chair to unscrew a vent.
Continuity mistake: When Monk takes the dog to the park, they're standing on the curb by the grass when he goes to take the leash off. After he says "get ready", the shot changes and they're not by the curb anymore. They're back on in the next shot.
Mr. Monk and the Sleeping Suspect - S2-E7
Character mistake: Brian Babbage is such a perfectionist and plans out such an elaborated plan for his crimes that it is very unlikely that he would simply leave the ketchup bottles sticking to the ceiling for the police to be found after he had timed the perfect glue for his plan. Plus, he even had staffers who already knew about the bottles and it would have taken him not more than a single order for the bottles to be removed. (00:23:50)

Continuity mistake: When Natalie's fighting with Harold's cousin on the roof she loses her shoes. A shot later, and they're back on her feet.
Mr. Monk and the Daredevil - S6-E7
Revealing mistake: Watch as Natalie fights with Harold's cousin on the roof. The same shot where she's barefoot, she's also got thicker hair, indicating a wig on a stunt double.
Mr. Monk Meets His Dad - S5-E9
Character mistake: At some point Monk asks his father what the device in the car is. His dad tells him it's a GPS. It's difficult to believe that our detective doesn't recognize the machine, even if it's probably another model: the whole plot of the episode 2x03, Mr. Monk Goes to the Ballgame, revolves around a tampered-with GPS.
Happy Birthday, Mr. Monk - S8-E9
Character mistake: Given how Monk is it seems unlikely he'd forget about being in a dumpster filled with garbage and be able to enjoy a birthday party, even if it's his own.
Mr. Monk and the Lady Next Door - S7-E12
Audio problem: Two words near the beginning of Monk and Marge's dinner conversation are audibly spliced. When Marge says "what does a", the word "a" is cut off at the beginning; and when Monk says "detective does" in his response, the end of the word "does" is cut off during the s. (00:15:15)
Mr. Monk Is on the Run (2) - S6-E16
Plot hole: When Stottlemeyer is confronted by the Sheriff for not going to the morgue there are 2 plot holes in that one scene: first it is impossible for that article to be in the morning paper since the coast guard "just fished him out" last night. And secondly an article, especially a long article like the one about the body in the bay would contain a physical description of said body. That is the reason it does not make sense to even ask the captain why he did not go to check the body to see if it was Monk when the body was tattooed and had red hair. (00:13:30)
Suggested correction: The article being there that fast is a bit odd, the sheriff mentions a police alert had been put out and was why he went to the morgue. That was what he was confronting the captain about. Even without the article the point of why he didn't check is still a valid question.
Do you really believe that the police alert did not include a description of said corpse being found for the missing person's unit to combine against their lists?
Mr. Monk and the Three Julies - S6-E13
Plot hole: There is no other reason for Randy to hand Natalie the walkie-talkie but to fill that plot hole. He is in fact in breach of police rules when he hands his department issued radio to a civilian to answer it while he is on duty. The same goes for Stottlemeyer and the car keys he hands to Monk. Even more so since he knows Monk could not even drive that car to the side in the far fetched scenario that a police car had to be moved while the cop is serving a warrant inside the building the car is parked in front of. Mr. Monk and the Airplane - S1-E13
Factual error: Though anti-freeze is lethal it does not kill quietly and fast but rather agonizingly, painfully and slowly, inducing vomiting and fever. (00:30:30)
Mr. Monk Gets Cabin Fever - S3-E12
Continuity mistake: When Natalie opens the captain's smashed cake in the van it is vanilla, but when she gives it to the captain he grabs some with his fingers to eat and it's chocolate with only a bit of vanilla. (00:28:40)
