Deliberate mistake: In the scene where JD is practising emergencies with a corpse, you can noticeably see the corpse's chest moving up and down - it's breathing.
Visible crew/equipment: When Ted walks past J.D. and the Janitor you can see the mic on top in middle of screen. (00:09:05)
My Day Off - S1-E9
Continuity mistake: At the end when they are in the bar, Dr Cox asks for 5 shot glasses, the camera cuts to JD and then back to Dr Cox. The glasses have been filled, but not enough time has passed for all 5 to have been done.
Continuity mistake: When J.D. marks on the wall, Its not the same mark when the Janitor says "You drew on the wall" the second time. The first mark has a curve near the top, The second one bends in the middle. (00:01:10)
Continuity mistake: The letter that JD drops on to Turk's tummy changes angle between shots.
Visible crew/equipment: When Dr.Cox leaves the room you can see the boom mic in the top of the window of door glass and then it goes up and disappears out of frame. (00:06:15)
My Last Day - S1-E24
Continuity mistake: Right at the end, when Dr. Cox turns to face JD there is a cup next to his right elbow, the shot cuts to further away, he gets up and storms off and the cup is no longer there.
Continuity mistake: When Jordan gets to Dr. Cox's place and explains how she got pregnant she says "Oh, the bus boy, the pool boy or something boy." Dr. Cox place both his hands on his face. When it cuts he's only got his left hand on his face.
Revealing mistake: When Kelso hits Dr Hendrick's bike with his car at the end, you can tell it is not Kelso. While you can't make out his face, you can see that the man driving has dark black hair, while Kelso has light brown/grey hair.
Revealing mistake: When Dan is in the tub you can see his dark swimsuit in a few shots.
Continuity mistake: In the episode where Elliot is missing J.D. and talking to him a lot on her phone, he asks her to drop the phone into Dr. Cox's pocket. She drops it into his left coat pocket, but he later pulls it out of his right coat pocket. He couldn't have changed it in his pocket, because he's surprised to find it.
My Choosiest Choice of All - S3-E19
Other mistake: When Carla goes to inject Irv with the syringe, she doesn't push the plunger down, but Irv still falls unconscious.
Continuity mistake: While Carla's brother eats his chips, his hands move from left to right while holding the bag through out the scene. (00:09:00 - 00:09:35)
Continuity mistake: When the guys duck into the water, the water level is near the top, but when the lady comes to the spa the water level is much lower. (00:09:15)
Continuity mistake: When the janitor is speaking into the stethoscope, his right elbow is on his knee and his left hand is on the mattress, when it cuts closer, his elbow is further up and his hand is next to his leg.
Continuity mistake: In the shot-reverse-shot between J.D. and Elliot at the end of this episode when they finally kiss, J.D.'s nose is on the left side of Elliot's face, and in the subsequent reverse shot it's on the opposite side. (00:20:00)

Continuity mistake: When Dr. Cox rushes into JD and Elliot's office saying "You are the worst Co-Chief Residents of all time," JD has his hands on the table. When it cuts he has them on his lap.
Revealing mistake: J.D visits Danni's apartment to talk to her about Elliot. The guy who answers the door says 'I better put on some underwear' but when he walks away you can see that he is wearing boxer shorts.
Answer: Dan was drinking beer and Cox feared that Dan might get so intoxicated he would fall asleep and drown in the tub.
