Other mistake: When Piper and Leo are talking to Phoebe about Phoebe's fear of accepting Cole's marriage proposal, Piper comments "...even though he could rip your throat out with his teeth", but the closed captions mistakenly read "with is teeth." (00:05:05)
Continuity mistake: Phoebe is making her coffee and pours cream in. The next scene it's almost overflowing and black.
Brain Drain - S4-E7
Revealing mistake: When the Chameleon Demon changes from a lamp to a demon he throws Piper and Phoebe with Telekinesis but Piper looks to the right before he motions the toss. (00:03:05)
Womb Raider - S4-E21
Other mistake: Piper tells Page that her doctor told her she probably wouldn't be able to conceive because of scar tissue from blunt force trauma. But Leo has healed Piper, if there was scar tissue after Leo heals, then all 3 girls should be covered in scars.
Suggested correction: They are thrown around quite a bit before even finding out Leo is a whitelighter. Also, he doesn't heal them every time they get thrown or hit in the abdomen. In fact, we usually don't see him heal them at all if they're just thrown across the room or punched in the gut, unless there's a visible wound.
Audio problem: When Piper, Paige and Phoebe run up to the attic to run from Shax, Piper says that they're about to find out if Paige is their sister, her mouth is shut when she is supposedly saying the sentence.
Visible crew/equipment: Just after Paige orbs into Phoebe and Cole's hotel room, a boom mic can be seen at the top of the screen.
Continuity mistake: In the scene with Paige and Jordan Bridges' character, nearer the end, when they are sitting on the couch and his possessed character leans in for a kiss, from the back shot, he leans in and his head is tilting towards his right shoulder (you see part of his profile and the back of his head and Paige's face); when they change the shot so you see the front of his face and the back of Paige's head, his head is tilted back towards his left shoulder.
Continuity mistake: When Piper is grieving Prue's death in the mausoleum, Leo's hand moves from cupping her face to resting on her shoulder blade between shots. (00:37:15)
We're Off to See the Wizard - S4-E19
Continuity mistake: When Leo arrives to tell the sisters about the wizard, Phoebe's hands change position between shots. (00:29:55)
Charmed and Dangerous - S4-E13
Revealing mistake: In the scene where The Source uses Piper's powers together on the clay object (it blows up and then freezes), after he unfreezes it, the pieces shatter and one lands on the stand thing. As the camera moves in towards The Source, before the camera passes the clay piece it visibly vanishes (as it is CGI) and then the Source holds up a piece which is much larger and a different shape. (00:19:45)
Size Matters - S4-E5
Other mistake: Since when are managers, on a trial basis no less, allowed to renovate and change the name of a club?
Suggested correction: Piper says in the episode that she allowed him creative control because the club wasn't doing well, and he was well-known to be good at what he does. It's made clear throughout the episode, by way of him telling her multiple times that if she doesn't like it that he'll get rid of it, that she still has full veto power on any changes.
Other mistake: Yenlo explains that he escaped limbo through the water in the clouds. And the yen master says there is no other water around. So why do the girls come in through a cave? If they have to leave through the water in the clouds they should enter through the clouds.
Womb Raider - S4-E21
Continuity mistake: When Phoebe gets a stomach cramp in Detective Miles' office, she doubles over in pain. In the next shot, she's standing up straighter than she was. Her right hand also moves from the chair in front of her to her stomach. (00:22:55)

Continuity mistake: When Phoebe talks to Morris in Detective Miles' office, her hands change position instantly between shots. (00:22:45)
Continuity mistake: In the recording that Agent Jackman plays for the sisters, the end of the reversal spell is heard "bring her back to me," but when Paige and Piper said it moments earlier, they only said "bring her back." (00:17:45 - 00:19:10)
Suggested correction: They say "bring her back to me" both times.
Continuity mistake: When Paige is telling Glen the evil enchantress story, her hand suddenly appears under her chin. (00:01:15)
Continuity mistake: When Phoebe receives the phone call from her doctor informing her that she is pregnant, she faints and starts to fall to the right. The next shot shows her lying down as if she had fallen to the left.
Other mistake: When Piper and Paige are talking at the Social Services building, Paige talks about one of her coworkers, but in the closed captions, the speaker is mislabled "Piper." (00:41:30)
