Continuity mistake: When the lift malfunctions, the lady has her head and arm outside the lift when she starts screaming to pull her back inside. We get one shot of her again with her head and arm outside before the shot changes to the security camera angle, and in this shot only her head is sticking out from the doors, with no time for her to move her arm (which was stuck) back inside the lift car.
