Continuity mistake: When Julia is first seen she is explaining the satellite images to a guy, and in one particular shot she holds the photograph near the bottom of it. Then in the following shot of the photo, her hand has moved up to the top of the photo. (00:18:25)

Continuity mistake: When the Russian Major is talking to the other lower ranking soldier just outside the train, in one particular shot the soldier is looking directly at the Major. Then in the following shot he is looking down at his feet, and lifts his head up twice to look at the Major again. (00:04:15)

Continuity mistake: When Tom and Julia are on the way to Vienna Tom changes seats to sit opposite Julia, and he puts his mug down with the mug's handle facing towards the plane's window. In the following shot the mug has turned around its handle is now facing Julia. (00:33:30)

Continuity mistake: When Tom's friend gets shot and the other guy starts to shoot at the windshield, the bullet holes change between shots. (00:46:35)

Continuity mistake: When Tom and Julia are in Julia's office, Tom sits down while Julia is on the phone and begins to read the small card from the flowers. In one shot Tom's three/four fingers are on the back of the card, then in the following shot he is holding the card at the edge with just his fingertips on the card. (00:26:55)

Continuity mistake: When Rogue is seen for the first time, her hand is moving up photographs on the wall, and if you look closely there are some kind of red poppies when she reaches the end, yet when it cuts to another shot a bit further away, her finger is a lot further down the wall from the red poppy/flower. Continuity mistake: When Howard Payne calls Jack in the bus and is saying he wants his money, etc, at the start of the next scene the bus can be seen with not only its front doors open, but the back doors are also open with a guy standing on the step in the doorway, even though the back doors haven't been opened yet. This shot is actually cut from the following scene, when Jack gets off the bus, the young guy from the bus stands in the doorway looking out at Jack leaving. (01:07:55)

Continuity mistake: When the Major and General are on top of the train just before they jump from one train to the other, the Major and General walk across the top and they pass one, maybe both, of the hijackers. When it cuts to another view, the hijackers are passing the Major and General again. (00:11:30)

Continuity mistake: When the hijackers are setting the nuclear bomb, they drill a device to the face of the bomb, and on the right of the screen there are wires sticking out of the little silver box. In the following shot one of the hijackers opens the little silver box and pulls the wires out again. (00:12:10)

Continuity mistake: When Frank is pulling down the gravestone with a chain, in the shot from the front, William's is to the left about a foot away and Maria's is to the right, the same distance. In the shot from behind the stone, there is another stone you can just about see on the right of the screen and it can't be William's because this stone, or something, is directly behind Frank's. Continuity mistake: Just before the two trains crash, there are 4 poles along the track and not far apart, then when the camera cuts to a wider shot, there are fewer poles and at a much greater distance from one another. (00:14:20)

Continuity mistake: At the end of the film when Frank Castle is standing over Howard, Frank grabs Saint's right leg (you can tell its his right leg by looking at the shoe), but in the following shot he is holding Saint by the left leg. (01:45:35)

Continuity mistake: When Frank crashes his car and Harry Heck pulls up in his, note the distance from Heck's car to Frank's car, perhaps only a foot, yet when it cuts to an overhead shot, Heck's car is now quite some distance away from Frank's without anyone moving it. (01:03:50 - 01:04:25)

Continuity mistake: When Frank walks up to Saint's son, Frank waives a white flag, but between shots his position changes from being really close to Bobby, to being at least a foot away and his body side on. (00:02:25)

Continuity mistake: When Saints's men are killing Castle's family, there is a shot of Frank and his dad shooting at one of the guys behind the barbecue stand. In one shot Frank and his dad shoot the salad bowl and most of the salad comes out, then in the following shot the salad has magically reappeared. (00:22:30)

Visible crew/equipment: When Howard Saint's men begin to chase after Frank's wife in their red 4x4, just after a couple of guys jump in the back, the camera pans forward to get a view of the driver. As the camera pans forward a boom mic with a large pole is visible in the reflection on the car. (00:24:20)

Continuity mistake: When the cops turn up at the firearms deal, someone says, "Your under arrest," and there are two to three cops. When Frank says his line, the camera cuts to an overhead shot and there are now 6 cops all grouped together standing only a few feet from Bobby, Frank and the others. (00:04:00)

Revealing mistake: In the scene where Jack has come back into the bus from being underneath it, he is talking to Annie about losing gas, if you watch closely he says "Mac" without pressing his button to call him first, then in another shot you see him press it when he starts talking to Mac. Continuity mistake: When Annie swerves to avoid the schoolteacher and the kids, in the previous shot of the kids and the schoolteacher, they have already stepped out onto the road and are crossing, yet when the camera cuts to a view looking out the bus the schoolteacher and the kids are just stepping off the curb and walking across.
