Visible crew/equipment: The shadow of the camera is visible on the car's hood when Al is trying to convince Mrs. Melanie not to go after Sam. It lingers there for a while in the shot.

Visible crew/equipment: When Malia points the gun to the dealer, for a second you can see two finger tips at the right edge of the screen. There is no other in the alley and it can't be the dealer because the angle would be wrong. (00:29:20)
Visible crew/equipment: When Lexy is scared in the classroom and stands up, there's a closeup of her eyes, and you can see a very clear reflection of a lighting bounce-card in her eyes.
Cartman Gets an Anal Probe - S1-E2
Visible crew/equipment: When Chef is singing: "I'm gonna make love to you, woman!" A crew member's hand can be seen moving the character's head. This was later fixed in the DVD version, but it was not fixed in the original version.

Prodigal Son - S5-E3
Visible crew/equipment: After Agent Hobbs reads the text that Zack sent to Clayton, Jubal tells ADIC Reynolds they need to evacuate Chapman, and in the next shot facing Reynolds there's an orange T-mark taped to the floor behind him. (00:19:35)
Visible crew/equipment: When Sydney taps the shoulder of the baddie who nearly disarmed Nigel, a strong light is cast on his face just before the cut. Nobody was supposed to point a flashlight or a torch at the fight. (00:38:15)
Visible crew/equipment: After Norman runs from the men that have him captive, Norman is dragged back to The Box. As he is being dragged you can see the mic cord across his stomach.
Visible crew/equipment: In episode 1, "Computer Logic", there's a scene in which Austin & Michelle step into a computer convention, where a hot-shot software designer, Mr, Blaine, is delivering a sales pitch from his wheelchair. In one shot the boom microphone can be seen dipping deeply into frame.
