Continuity mistake: The mud stain on Harvey Dent's suit disappears when he assaults the henchman that kicked the mud on him and then reappears when he is walking away from the scene. (00:04:50 - 00:05:50)
The Arrow That Is Not Aimed - S3-E13
Visible crew/equipment: Fukuda-san is not impressed by Magnum making excuses for Tozan. When Tozan says "There is nothing to say. Fukuda-san is right", set lights are reflected in Tom Selleck's sunglasses. (00:27:00)
A Good Clean Bust with Sequel Rights - S5-E6
Visible crew/equipment: While Falcone is with Seales, the reps from the consumer groups confront Falcone about his toys. When Seales says, "Our toys have a psychological counter-violent quality," in the next shot just as Seales starts to walk toward the reps, we can see the head of a crouching crew member moving in front of Falcone. Continuity mistake: When coming out of the sea at the start, the guy's hair goes from sleek to messy and back between shots. (00:02:00)

Revealing mistake: When Michaela and Jared arrest Cheryl, Jared takes the money out of her bag and you can see the "For Motion Picture Use Only" designation on the bills. (00:32:00)
Mr. Monk and the Very, Very Old Man - S2-E5
Visible crew/equipment: In the scene where Captain Stotlemeyer is putting one of the large snakes back in the glass container, he turns and the base of his wireless microphone is visible when his jacket gets bunched up.
Continuity mistake: Shiro cuts Stacey's right cheek with his sword, drawing blood. However, in subsequent shots, Stacey's cheek is completely unmarked, and there is no sign of any cut or blood. (00:32:46 - 00:33:58)
Continuity mistake: At the beginning, the way Holmes holds the photo is different in consecutive shots. (00:01:00)
Continuity mistake: 1.08 (Depraved Heart) Lightman is working late and his daughter drops by to bring him Thai food. As she's walking towards her father, her arms are crossed. Cut to the immediate shot from behind, and her arms are down at her sides.
Other mistake: When Norman and Madeleine are talking outside the Motel about Sam, the left side of the screen appears to be split down the middle after Norman says "Because you know it's safe to tell me", causing a weird effect on the chair that Madeleine is sitting in.
Factual error: When Debbie Harry's "I Want That Man" is playing, just before Deb is shown being dropped off at school by Gio, an overhead shot is shown of basketball courts under a flyover. You can see modern cars parked under the flyover: at least 20 years more modern than in 1991 when the show is set. (00:06:57)
Continuity mistake: Early on, Miss Fisher and Doctor McMillan are strolling among the dockers in the background. But Miss Fisher hasn't disembarked the ship yet, that happens in the following scene. (00:01:45)

Visible crew/equipment: When Mark Harris shows Matlock into his office, the actors' three tape marks (one T-mark) are visible on the carpet behind the chair, and at the bottom of the screen.
The Debate - S6-E12
Visible crew/equipment: After Alicia and Frank have begun their mini debate in the kitchen, the hotel employee enters the kitchen; then, as Alicia says, "The system needs to be tweaked, not overhauled," the camera rounds the corner, and we can see a red T-mark and a red tape marker on the floor. Glory Days - S8-E3
Continuity mistake: Towards the end of the episode, during the final shootout, Murdoch ambushes an armed bad guy in the attic and jumps through the wooden wall with him. As they fall down, Murdoch's trademark hat spectacularly flies off his head, but in the next overhead cut - showing him and the bad guy lying on the ground - the hat is on his head again.
Chapter One: Suzie, Do You Copy? - S3-E1
Continuity mistake: In Bruce's hamburger the salad is on the bottom and the bacon is on the top, in the next shot the salad is on the top and the bacon has disappeared, replaced with ketchup. (00:23:05 - 00:27:10)
Visible crew/equipment: As Hannah goes to take the cassettes we can see the shadow of the moving camera. (00:04:25)
Revealing mistake: The scene is set in Boston, MA, USA, but at a murder location, a European-style circular power outlet is visible through the glass doors at the bottom of the concrete wall. (00:01:20)
