Audio problem: When Hardison is talking to Carrey, just after he names his price ("30%"), there is a shot from a strange angle in which we hear Hardison talk while his mouth doesn't move at all.
Audio problem: When Oliver is fighting the guys in the hallway, he shoots the second to last guy. But when he starts to draw an instant beforehand, you can see that his right hand, with which he holds the arrow, is nowhere near the quiver when the sound of the drawn arrow begins.
Audio problem: When Gemma is talking to Clay in jail there's a shot "en profil" in which we only see the silhouette of her face. When Clay sits down you can see she's saying something, while the only sound we hear is Clay exhaling.
Audio problem: When Carter is searching Jean Jackson's house she asks him if he has a warrant - although the camera isn't on her you can see her reflection in the mirror and you can see that her mouth doesn't open when she is asking Carter this.
Audio problem: The director can be heard saying "Action" at the start of the scene. (00:16:35)

Audio problem: When Easy are attacking the 88 battery, in the scene where Buck scouts out what's ahead of him before throwing grenades, you can see a soldier holding an MP40 (machine pistol), who fires one shot with the sound effect of a Kar98 rifle. (00:31:50)
Santa's Little Helper - S1-E10
Audio problem: When Amy is playing the piano with Morgan, the last few notes that they play while the camera is focused on them are out of sync. (00:19:50)
Audio problem: After Nasedo reveals himself in the morgue, there is a shot on Michael's face. There is no sound, but Nasedo can be seen in the foreground either talking animatedly or having an extremely localized seizure. (00:23:55)
Episode #3.3 - S3-E3
Audio problem: In a conversation with Colin, Zoƫ says "What is the point of it being able to do all this stuff if we can't see the results?" The camera then cuts away from her face, she continues "We need to find out who [name I won't embarrass myself by misspelling] is talking to." This line has been dubbed in, as it is said while the actress' face is off-screen and in a completely different tone of voice. (00:35:30)
Audio problem: When Becky runs down the stairs and yells "Mom!" you can hear the echo throughout the sound stage.
Audio problem: Just before Avon reveals Orac is on the shuttle, you can hear his operating noise. If we can hear it, so should Vila.

Audio problem: In the last scene of the episode Sheridan says to Mr. Sebastian "The morning after the last of a string of murders in the East End." His lips however, say West End instead of East End. The script and initial broadcast used the words East End, but after it was pointed out that this was factually incorrect, the line was dubbed over for all subsequent broadcasts and releases. Audio problem: While Ted and Jason are slopping pigs at the pen, Ted asks, "They really eat this garbage?" then as Ted pours the bucket contents we hear Ted exclaim, "They really eat this garbage!" but his mouth appears to say, "They really eat this crap!" (00:18:35)
Audio problem: When Olivia calls Astrid to tell her it wasn't Walter's research, we cut to Astrid who says, "Good news", but her lips do not move. (00:33:57)
Audio problem: When Barney and Willy are digging their tunnel toward the prison basement, Barney's voice and lip movements are badly out of sync. (00:31:05)
Audio problem: When Carcetti is talking to the Interdenominational Ministerial Alliance, there is a moment of silence after he announces that he'll treat them fairly regardless of endorsement. During this moment of silence, the shot changes to an angle where Carcetti is mid-speech, while the audio is silent. The scene resumes in sync with the audio as soon as the shot changes. (00:47:20)
