The Fraudulent Volcano - S1-E16
Audio problem: Simon One's mouth does not move when he is giving the fire commands to his gunner to knock down Race and Dr. Quest's plane.
Midori no chikyû wa makasetoke - S1-E2
Audio problem: In the hangar scene at the beginning, Goat is talking to Akito. Seiya Uribatake in the background is arguing with his fellow mechanical worker about the reparations to the Aestivalis. The conversation happens in the back of the hangar and it is meant to be strong background noise, annoying enough to prompt Goat's reaction, but in the English dub it is so strong that quite literally most of the main dialogue is nearly inaudible because of it. The dubber also adds a surprised/scared "waah" at the end when Goat cuts him off, but that is incredibly delayed compared to the reaction of the character on screen. (00:02:35)
Coming Home - S1-E13
Audio problem: The first refugee to greet Amuro has a male voice (original version, but also English dub), but when you look at the figure, it's kinda rounded in the chest and in the next shot when just the legs are in sight, just match the positions of those around Amuro and it's quite clear that 'he' is the one wearing a skirt. (00:10:00)

Onna no koi wa snesouyo! Kakutou shintaisou de iza shoubu - S1-E12
Audio problem: When Ranma and Akane close the door behind them avoiding the gas trap, the English dub adds a lot of coughing, even when they have their mouth closed. (00:16:40)
Audio problem: In "The Robot", Gumball whimpers while his mouth is open. He whimpers again, and this whimper sounds like the first one, except his mouth is closed.
The Planet of Dr. Moreau - S1-E4
Audio problem: At the very beginning of the episode, the Silver Surfer emerges from a wormhole screaming "No." If you look closely as he is approaching the camera, his mouth is closed until just before he slows his approach.
The Prison Prep School Murder Case File 3 - S1-E12
Audio problem: Someone "accidentally" drops an eraser, drawing the teacher's attention. It's a soft pencil eraser with no additional parts, but it falls with a clicking noise as if it were made of hard plastic. (00:08:30)
The Creepy Heap from the Deep - S2-E8
Audio problem: When speaking about the hidden beauty parlor in the submarine Velma's voice comes from Daphne's mouth briefly.
The Man They Called a Magician - S1-E2
Audio problem: Jigen shoots Pycal in the head with no results. He says that his gun is useless, but his lips don't move. (00:05:50)
Audio problem: In the final scene when Cleveland says to Terry "I'm gonna get paid and you're gonna get laid", they both start cheering. But when it comes to the wide shot, Terry is still cheering but his mouth isn't moving. (00:19:55)
Guilmon Comes Alive - Part 1 - S3-E1

Audio problem: In the park where Bobby is face to face with Chickiebutt and his Acme Mega-Beak, Bobby says, "He's wearing the Acme Mega-Beak over here" but his mouth makes no movement as he talks.
Beauty and the Obese: Part 1 - S1-E1
Audio problem: After the bartender himself offers a lap dance to the man who just talked to Persephone, the man walks off to the right of the screen. The bartender offers him a cheaper price when the camera cuts to a different angle after he walked away. At this point, the man would now be just off the left side of the screen, but when you hear him say "No" it comes from the right side when listening with headphones. (00:02:42)
Audio problem: When Princess Toadstool jumps into the Warp Pipe, she speaks, but her mouth does not move. (00:06:11)
