Confinement - S2-E5
Continuity mistake: Ruby asks Ash where the the Kandarian Dagger is. He says that it was confiscated by the police. In the previous episode, she had and used it, and they have not interacted in the intervening time period.
Factual error: In scenes from the oil rig there are shots that show the shore line. This oil rig is supposed to be offshore of Louisiana. However, mountains can be seen in the shots of the shoreline. There are no mountains in Louisiana.
Continuity mistake: After Buffy finds Morgan's brain, Willow pulls up Morgan's school file on one of the library computers without even touching the keyboard. One moment the monitor is on the desktop, the next it's showing Morgan's file.
Plot hole: Hippolyta tells Hawkgirl of Aresia's background of being born in the outside world, the war in her country and right up to her drifting to the shores of Themyscira. It would be impossible for her to know anything of Aresia's background as in part two, it's explained that the captain of the ship who saved Aresia died shortly after reaching Themyscira and therefore would not have been able to tell Hippolyta anything.
Communication Breakdown - S5-E19
Character mistake: When Mac, Stella, and Danny are trying to determine where the fired bullet entered the train car, none of them are wearing rubber gloves. But even though they're not handling any evidence, they're still processing a crime scene. (00:05:00)
Character mistake: When Matt is talking to his priest Father Lantom, he says "it was a nice sermon" instead of "it was a nice homily." In the Catholic church "homily" is the correct word; sermons are Protestant. (00:11:30)
Factual error: Wanda's father opens his briefcase of DVDs and one of them is Malcolm in the Middle. But the bomb that killed him fell in 1999, and Malcolm in the Middle didn't start airing until 2000.
The Bridge - S1-E10
Revealing mistake: Agent Coulson turns the steering wheel several times despite the car being on a straight street.
Visible crew/equipment: When one of the guards cries "Destroy it!" watch to the left of Karga and you'll see a crew member in jeans hiding behind a wall. This was later edited out on Disney+. (00:18:54)
Factual error: A character in the episode has invented denim, but denim was invented in 1873 and was being used widely up to the time period of the series, which is set in 1893.
Suggested correction: The entire series was set in an alternate history of sorts, so most, if not all, historical "inaccuracies" cannot be regarded as mistakes.
Archer Vice: Southbound and Down - S5-E5
Factual error: When the bikers are chasing the bus, one of them crashes into the "Now Leaving Texarkana" sign. However, shortly before this, we see Cyril crashing into the sign reading "Austin. 105 miles." Texarkana is almost 350 miles from Austin.
Revealing mistake: When Brad wears the sunglasses and holds the pole, you can easily see the string that holding it up.
Factual error: Calleigh Duquesne frequently wears completely inappropriate clothing throughout the whole series. She often wears blouses that are so low cut that the neckline is below the lower curve of her breasts. No officer of the court in the US would be allowed to dress this way. There is no grey area here, and this is not a character mistake - first time a CSI turned up at work dressed like that, she'd be sent home to change. Second time, she'd be on suspension until she agreed to change her dress standards. Also (and this regularly happens in CSI: NY as well) she is regularly wearing high heels - also a certain no-no for as CSI. Incidentally, male CSIs have equally strict dress standards.
Lehu a Lehu - S6-E2
Visible crew/equipment: Danny is supposed to be looking for the suspect in the woods by himself and you can see another person on the top right hand side of the frame. The crew member looks as if he knows he is in the way and slowly backs away. (00:35:15)
Audio problem: When they are trying to escape Koopa and the little caveman speaks, then Luigi answers him, he has the Princess' voice.
Character mistake: Previous episodes have established this story arc to be occurring in the year 2154. Yet here, when T'Pol records her log, she cites the date as 2152. (00:32:00)
Continuity mistake: When the Hulk throws the tire during his second transformation, the first shot shows a horizontal trajectory but the second shot shows a 45 degree angle trajectory, which makes the tire land much further than expected.
Continuity mistake: When Crockett tries to stop a rapist in a white Lamborghini the Lamborghini's taillights switches design during the chase.
Visible crew/equipment: During the final chase, Jaclyn Smith's double stuntwoman is extremely evident. (00:43:45)
Continuity mistake: Season 4, episode 10 "Ben 10 vs. The Negative 10, Part 2": When the Forever King makes his announcement "Nothing will stop this little home-coming", he stands outside the Mount Rushmore base on bare soil. But in the scene where Max reviews the recording of this speech, all of a sudden we find the Forever King standing on a neatly tiled floor.
Suggested correction: Theoretically correct, but it can be argued that the only evidence present is the bullet (which has DNA and prints burnt off when fired) and the body (which can only be handled by the MEs) gloves are not required.

What utter rubbish. No CSI would enter a crime scene without first putting on latex gloves - it is a carved-in-stone rule.
Exactly. Since Mac, Stella and Danny were processing a crime scene, they would need to wear gloves because without them, they would have contaminated a crime scene by not only getting their fingerprints over everything but, if one of them got a cut or scrape, their DNA would also be thrown into the mix and any defense attorney can call them out on their carelessness and have any evidence thrown out.
