Suggested correction: It's not a blood pack. It's Karl's blade.

Kissed by Fire - S3-E5

Continuity mistake: At this point, Jamie Lannister has lost a hand at the, well, hands of his captors. As they shove him to his knees in front of Lord Bolton, he uses both of his hands, with no stump being visible. (00:12:39)

The Rains of Castamere - S3-E9

Continuity mistake: At the climax of the Red Wedding massacre, Robb crawls towards his dead wife Talisa from the direction of the doors, which are at the opposite side from Walder Frey's throne. Robb turns Talisa onto her back from her left side (ie. he is on the right hand side of her body), and looks down at her face. This means that Talisa is lying in such a way that, if facing Walder's throne, her head is on the right and her feet on the left, towards where Catelyn is holding Walder's wife hostage and pleading for Robb's life. However, when Walder refuses, Robb has changed position and is now on the left side of Talisa's body, which is presumed to be facing the same direction as before as there is no reason for it to have changed. Robb then turns towards Catelyn who is still on the left side of the room (if facing from The Door), and is stabbed by Roose Bolton. As Robb collapses to his death, looking directly at Catelyn, he lands next to Talisa, whose head is now closest to Catelyn with her feet facing the other way, meaning that her body has somehow rotated by 180° in a matter of seconds. This is an especially bad mistake considering that several people have claimed the Red Wedding was completed in one take, which is not possible if this error exists unless Oona Chaplin for some reason decided out of boredom to play spin the bottle with her own body while out of shot, which would have interfered with Richard Madden's position anyway. Two Swords - S4-E1

Continuity mistake: When Arya and Hound come upon the murdered people Arya is riding facing left. After they pass, she is facing right. (00:48:40)

The Mountain and the Viper - S4-E8

Continuity mistake: When Oberyn first tells Gregor that he will make him confess to raping and killing his sister Elia and her children, he is holding his spear with both hands. In the next shot, he's holding it with just one hand.

The Sons of the Harpy - S5-E4

Continuity mistake: When the Faith Militant descend on Ser Loras Tyrell's training session there is a shot after the Militants have come down the stair of Loras fighting from behind a man's shoulder, in this shot you can see they are fighting on grass surrounded by walls with sheer drops with no entry point other than the stairs the militants walk down. In the next shot you can see the 9 militants that have come down the stairs approach Loras from one side, but suddenly 6 militants join him from the opposite side, where they definitely were not before. In the zoomed out shot you can see there is nowhere for these men to have come from. (00:11:15 - 00:11:50)

The Sons of the Harpy - S5-E4

Continuity mistake: Towards the end there is a body of a dead Son of the Harpy lying between the fallen Unsullied and the side corridor visible in the wall, located very close to either. In the next show the body is gone. Also, as the camera angle changes, the blood stains on the walls look differently. (00:48:50)

Unbowed, Unbent, Unbroken - S5-E6

Continuity mistake: When the Sand Snakes attempt to kidnap Myrcella, as Nymeria Sand runs up to Myrcella to seize her, she is holding her large whip in her right hand. However, in the very next shot, the whip has disappeared, and Nymeria is now holding a dagger in her hand instead.

Kylantha

The Gift - S5-E7

Deliberate mistake: After Ramsey shows Sansa her flayed northern friend, when it cuts to Davos riding the horse through the encampment one of the shots is flipped. Note his sword and his cloak's crossed straps have switched to opposite sides.

Super Grover

The Dance of Dragons - S5-E9

Continuity mistake: In the scene in the amphitheatre, while seated in the royal box, Daenerys is wearing a full length white gown with flat pumps and an ornate silver necklace with a large dragon motif on the left side. During the revolt when she is seen running across the fighting pit the dress is shorter and she seems to be wearing some sort of leggings underneath it. Daenerys escapes on the back of her dragon and when we next see her in episode 10 she is wearing a completely different knee length pale blue gown. The necklace also seems different with her hair spread out on her left shoulder to disguise the lack of the dragon motif. She is also wearing grey boots with a fairly high heel. Obviously dragons come with a secret baggage compartment.

The Broken Man - S6-E7

Continuity mistake: In one shot Sansa is writing a letter to Petyr Baelish, and is pressing a seal onto hot wax on the lower right corner. The next shot Sansa raises the letter, and the wax is gone. (00:39:45)

Uri Raz

No One - S6-E8

Other mistake: The faith militant attacks Gregor Clegane. Gregor chokes him. You will see that before the faith militant drops his weapon with his left hand, both of his hands are already holding Gregor's massive arm. (00:13:05)

Battle of the Bastards - S6-E9

Continuity mistake: When Jon Snow is all alone in the field drawing his sword to fight the incoming enemy cavalry, behind him we see an empty field. but seconds later as the enemy cavalry is near him they are met by Jon's own cavalry from behind. (00:37:30 - 00:38:20)

lionhead

Battle of the Bastards - S6-E9

Character mistake: When Theon and Yara are given an audience with Daenerys, Tyrion remarks to Theon that when they last saw each other at Winterfell, Theon was making jokes about Tyrion being a dwarf. The mockery was actually the other way around: Tyrion mocked Theon for being a glorified and subservient hostage of the Starks, as well as taking potshots at the foolish strategy of the Greyjoy rebellion.

Show generally

Question: Why were so many actors in this show recast? If the showrunners knew they would be using certain characters for multiple seasons, then why not lock them in long term?

Phaneron

Chosen answer: Considering the show was in production for nearly a decade, it is hardly surprising that many characters were recast. Actors constantly move on to other jobs, creating scheduling conflicts. Some may not want to be tied to one project for so long a period, particularly for relatively minor roles that paid little money. As some characters became more prominent, the producers did recast a few roles with better-known actors. For example, Max Von Sydow replaced the actor who originally played the Three-eyed Raven. Some actors voluntarily left for personal reasons. Jennifer Ehle was originally cast to play Catelyn Stark but dropped out because she'd recently given birth. Other actors claimed they were replaced for "political" reasons. Here is a website with a rundown of which actors were replaced and why: https://screenrant.com/game-thrones-characters-recast-new-actors-every-why/.
