Back to Battle City: Part 3 - S3-E27
Continuity mistake: At the start of the episode, in the wide shots of the duelling room, the platform that Yugi is standing in is several meters below the number 2000. This is because Yugi has 3000 life points, and the platform's position depends on what the duelist's life points are. When Marik says to himself, "Crushing the Pharaoh is a much more appealing choice," the platform Yugi is standing on is now aligned with the number 2000 (which shouldn't be the case since his life points are 3000). (00:02:13 - 00:02:52)

The Darkness Returns: Part 1 - S3-E28
Continuity mistake: When Tristan attempts to leave the tower to tell Serenity about the next duel, Mokuba calls out to him. The wall next to Mokuba has nothing on it, but in the next shot, a phone has suddenly appeared on the wall, which Mokuba then uses. (00:01:51)

The Darkness Returns: Part 1 - S3-E28
Continuity mistake: On Joey's first turn, after drawing the Alligator Sword card from his deck, he looks at the other five cards in his hand. At first, his hand is at a horizontal angle, but in a close-up of the cards, his hand is at a vertical angle. (00:07:56)

The Darkness Returns: Part 1 - S3-E28
Continuity mistake: When Joey stomps his foot on the floor, a line can be seen beneath his foot, showing where the tiles were placed. In the next shot, the line on the floor disappeared. (00:17:03)

The Darkness Returns: Part 2 - S3-E29
Continuity mistake: When Baby Dragon is on Joey's field, the monster card on his duel disk is positioned at a rotated angle with the artwork facing to the left and the wording facing to the right. But later in the episode, when Joey summons Jinzo, in the shot of the Jinzo card being placed on the duel disk, the Baby Dragon card is facing the opposite direction, with the artwork facing to the right and the wording facing to the left. (00:05:37 - 00:13:07)
The Darkness Returns: Part 3 - S3-E30
Continuity mistake: When Marik draws the Machine Duplication card from his deck, he has three fingers pressed against the back of the card. When it cuts to a close-up of the card, he only has one finger pressed against the back of it. (00:05:27)

The Darkness Returns: Part 4 - S3-E31
Continuity mistake: When Roland says that Joey is disqualified, his right arm is raised and pointed to the right. In the next shot, his right arm is lowered. (00:06:45)

The Darkness Returns: Part 4 - S3-E31
Continuity mistake: When the Millennium Necklace is glowing while in Yugi's hand, in the shot where he says, "Maybe the Millennium Necklace is telling me Joey's gonna be okay," his fingers are spread apart. When the shot cuts, his fingers are closed together. (00:15:16)

Clash in the Coliseum: Part 1 - S3-E32
Continuity mistake: When Kaiba draws his Obelisk the Tormentor card, he is holding it with two fingers against the back. One finger is close to the bottom of the card and the other is closer to the middle, but in the next shot, his fingers are nearer to the top. (00:06:35)

Clash in the Coliseum: Part 2 - S3-E33
Continuity mistake: Just after Obelisk the Tormentor has been summoned to the field, in the shot of Yugi saying to himself, "I hope I can hold my own against the power of Obelisk," his right forearm is raised and his hand is closed into a fist. When the shot cuts, his forearm is lowered, and his hand is not closed into a fist. (00:19:26)
Clash in the Coliseum: Part 3 - S3-E34
Character mistake: When Slifer the Sky Dragon's attack points go from 2000 to 3000, Kaiba tells Yugi, "But now that your turn is over, Obelisk regains its full strength," but Yugi's turn isn't over; it only just started, as shown by him activating Pot of Greed shortly after. (00:05:08)

Clash in the Coliseum: Part 3 - S3-E34
Continuity mistake: Just after Kaiba activates Command Silencer, in the shot of Yugi asking "What is that?", his hands are lowered, but in the next shot, his hands are on his ears. (00:05:53)
Clash in the Coliseum: Part 3 - S3-E34
Character mistake: When Kaiba activates Card Guard, he says he can discard two cards from his hand to protect his monsters from Yugi's attack. However, he only sends one of the cards in his hand to the graveyard instead of two. (00:08:57)

Clash in the Coliseum: Part 3 - S3-E34
Continuity mistake: When Kaiba realises Yugi was able to avoid losing the duel by discarding Kuriboh, he says, "How dare you defy the might of Obelisk with that puny Kuriboh," with his right forearm raised, while his hand is closed into a fist. When the shot cuts, his forearm is lowered. (00:11:17)
Clash in the Coliseum: Part 3 - S3-E34
Continuity mistake: When Kaiba tells Yugi that he will have to discard half of the cards in his hand, Yugi is standing upright. But in the shot of Kaiba explaining that Slifer's attack points are reduced by 3000, his torso is suddenly leaning to the right. (00:12:17)
Clash in the Coliseum: Part 3 - S3-E34
Continuity mistake: Yugi shows Kaiba the Electro Magnetic Turtle card that was returned to his hand from the graveyard. Then, in a close-up, he is shown turning the card around and asks Kaiba if he recognises it, even though the card was already facing him. (00:15:50)

Clash in the Coliseum: Part 3 - S3-E34
Continuity mistake: When Yugi removes the Electro Magnetic Turtle card from his graveyard, in the shot of him holding up the card so that he can show it to Kaiba, it is shaded yellow. But in a close-up of the card, it is shaded orange. (00:15:50)

Clash in the Coliseum: Part 3 - S3-E34
Revealing mistake: When Yugi holds up the Electro Magnetic Turtle card, which he has just removed from his graveyard, there are two other cards in his left hand that appear to be missing the illustrations for a brief moment. (00:15:50)

Clash in the Coliseum: Part 3 - S3-E34
Revealing mistake: When Yugi is telling Kaiba about his Electro Magnetic Turtle card, whilst he says "and it has quite a unique special ability," he lowers his hand and the card appears to be missing the illustrations. (00:15:56)
Clash in the Coliseum: Part 3 - S3-E34
Character mistake: When Yugi discards the last card in his hand after activating Spell Textbook, Slifer the Sky Dragon's attack points go from 1000 to zero. This makes Kaiba say, "Now that Slifer's attack points are zero, you'll lose the duel." However, at this point, Yugi has 3000 life points and Kaiba's Obelisk has 4000 attack points, so Kaiba still would have been able to win the duel if Slifer had 1000 attack points, as there would have been a 3000 point difference between the two monsters.
